摘要 目的探究基于信息-动机-行为技巧(IMB)模型的健康教育在永久性膀胱造瘘患者中的应用效果.方法选择2015年7月至2017年3月本院门诊就医就诊携带永久性膀胱造瘘管的男性患者108例,随机将所选患者分为试验组和对照组,各54例.对照组患者实施传统的健康教育,试验组实施基于IMB模型下的健康教育,随访1年,分别评价干预前后自我效能及自我管理行为,统计两组干预后患者并发症发生情况及干预前后生活质量的变化.结果试验组干预后自我效能(症状管理、疾病共性管理)及自我管理行为(认知性症状管理、与医生的沟通)得分均明显提高,与同组干预前比较,差异均有统计学意义(均P< 0.05),与对照组干预后比较,差异均有统计学意义(均P<0.05);两组出现的并发症有堵管、结石、感染、膀胱痉挛,干预后试验组并发症的发生率(7.41%)及未按时换管率(7.41%)均明显低于对照组(37.04%、31.48%),均P< 0.05;SF-36量表测试结果显示试验组干预后各功能评分均明显提高,生活质量得到明显改善,与同组干预前比较,差异均有统计学意义(均P< 0.05),与对照组干预后比较,差异均有统计学意义(均P< 0.05).结论 IMB模型下健康教育模式有利于提高永久性膀胱造瘘患者的自我效能及自我管理行为,减少患者并发症的发生,提高患者的生活质量,值得在临床推广使用. Objective To explore the effect of applying health education based on the model of information-motivation-behavior skills (IMB) in patients taking permanent cystostomy.Methods 108 male patients with permanent cystostomy treated at the outpatient department of our hospital from July,2015 to March,2017 were selected,and were randomly divided into an experimental group and a control group,54 cases for each group.The control group took traditional healthy education,while the experimental group took the healthy education based on IMB model.Both groups were followed up 1 year.The self-efficacy and self-management behaviors were evaluated before and after the intervention,and complications and life quality changes before and after the intervention were statistically analyzed.Results The scores of self-efficacy (symptom management and disease common management) and self-management behavior (cognitive symptom management and communication with doctors) after the intervention in the experimental group were significantly improved,and were significantly different from those before the intervention (all P < 0.05) as well as from those in the control group (all P < 0.05).The complications occurred in the two groups included tube blockage,calculi,infection and bladder spasm,and the incidence of complications and the delayed tube replacement rate were significantly lower in the experimental group than in the control group (7.41% vs.37.04%,7.41% vs.31.48%,both P < 0.05).The SF-36 scale test results showed that the functional scores and the quality of life after the intervention in the experimental group were significantly improved,and were significantly different from those before the intervention (all P < 0.05) as well as from those in the control group (all P < 0.05).Conclusions The healthy education model based on IMB is conducive to improving the self-efficacy and self-management behavior of patients with permanent bladder fistula,reducing the occurrence of complications and improving their quality of life,so it is worth for clinical promotion.
