摘要
目的观察血栓通联合SolitaireAB支架介入取栓术治疗急性脑梗死的疗效。方法选取90例满足条件的缺血性脑卒中患者作为研究对象,根据倾向性匹配的方法分为常规介入取栓组(A组)、SolitaireAB支架取栓组(B组)及血栓通联合SolitaireAB架取栓组(C组),比较治疗后各组的mTICI、mRs、NIHSS评分和并发症或不良反应情况。结果 C组的mTICI评分明显高于A、B组,mRs评分则明显低于A、B组,差异均有统计学意义(P<0.05);随着溶栓后观察时间的延长,A、B、C 3组患者的NIHSS评分均有一定程度的下降,且以C组下降更为明显;溶栓后90 d,C组患者的NIHSS评分明显低于A、B组(P<0.05)。C组的血管再闭塞发生率明显低于A组(3.2%vs 22.2%,P<0.05)。结论血栓通联合SolitaireAB架取栓术对急性脑梗死患者有明显的改善作用,能够促进患者神经功能的恢复,并降低血管再闭塞的发生率。
Objective To investigate the efficacy of Xueshuantong combined with Solitaire AB stent thrombectomy in the treatment of acute cerebral infarction.Methods A total of 90 cases with acute cerebral infarction meeting the requirements were included in the study,and divided into three groups according to orientation matching methods: Conventional Intervention Group (Group A),Solitaire AB Stent Thrombectomy Group (Group B) and Xueshuantong combined with Solitaire AB Stent Thrombectomy Group (Group C).The three groups were compared in terms of the mTICI score,mRS score and NIHSS score as well as complications / adverse reactions.Results Group C had the mTICI score significantly higher than Group A and B and had the mRs score significantly lower than Group A and B,and the differences were all statistically significant (P<0.05).With the extension of observation time,the NIHSS score of Group A,B and C all decreased to a certain extent,and Group C had a more significant decrease;at 90d after thrombolysis,the NIHSS score of Group C was significantly lower than that of Group A and B (P<0.05).Group C had the incidence of vascular re-occlusion significantly lower than Group A (3.2% vs 22.2%,P<0.05).Conclusion Xueshuantong combined with Solitaire AB stent thrombectomy can improve the recanalization effect of occluded blood vessels in patients with acute cerebral infarction,promote the recovery of neurological function,and decrease the incidence of vascular re-occlusion.
