3.0T磁共振扩散张量成像在鼻咽癌颞叶放射性损伤的应用研究 被引量:1

摘要 目的:探究3.0T磁共振扩散张量成像在鼻咽癌颞叶放射性损伤的应用效果。方法:研究对象为本院2015年4月-2016年10月收治的72例行放射治疗的鼻咽癌患者,给予3.0T磁共振扩散张量成像检测,评估其在鼻咽癌颞叶放射性损伤中的应用价值。结果:第1组的FA值较对照组下降且差异有统计学意义(P<0.05),第2、3组的FA值高均于第1组且差异均有统计学意义(P<0.05);对照组与第2组比较差异有统计学意义(P<0.05),与第3组比较差异无统计学意义(P>0.05)。与对照组对比ADC值随放疗时间的延长呈逐渐下降趋势。放疗各组较对照组最大本征值λ_1明显下降,且其变化趋势与FA值接近,为第1组较对照组明显下降(P<0.05),第2、3组较第1组有所回升但仍略低于正常对照组。四组MD值比较差异均无统计学意义(P>0.05)。FA值第1、2、3组扩张张量值的变化幅度分别为3.81%、2.96%、2.33%,ADC值分别为2.00%、2.41%、3.66%,本征值λ_1分别为7.40%、4.15%、2.00%,MD值分别为1.07%、2.35%、1.55%,其中本征值λ_1第1组变化幅度最大。结论:将3.0T磁共振扩散张量成像应用于鼻咽癌颞叶放射性损伤中时,可反映放疗后颞叶白质水分子的弥散特点,了解其修复过程的变化规律,为临床提供理论依据,有助于监测放射线的损伤程度。 Objective:To study the value of 3.0T diffusion tensor imaging in radiative temporal lobe injury of nasopharyngeal carcinoma.Method:72 nasopharyngeal carcinoma patients treated by radiotherapy in our hospital from April 2015 to October 2016 received were selected as the objects,they were given 3.0T diffusion tensor imaging,its application in radiative temporal lobe injury of nasopharynx cancer were assessed.Result:The FA value of the first group was lower than that of the control group(P<0.05),the FA value of the second and third groups were higher than that of the first group(P<0.05),the difference between the control group and the second group was significant(P<0.05),but there was no significant difference between the control group and the third group(P>0.05).Compared with the control group,ADC value decreased gradually with the prolongation of radiotherapy time.Compared with the control group,the maximum eigenvalueλ1 of each group decreased significantly,and its change trend was close to FA value.The first group decreased significantly compared with the control group(P<0.05),while the second group and third group increased slightly compared with the first group,but still slightly lower than the normal control group.There was no significant difference in MD values among four groups(P>0.05).The change ranges of FA value in the first group,second group and third group were respectively 3.81%,2.96% and 2.33%,ADC value were respectively 2.00%,2.41% and 3.66%,eigenvalue λ1 were respectively 7.40%,4.15% and 2.00%,MD value were respectively 1.07%,2.35% and 1.55%.The change range of eigenvalue λ1 in the first group was the largest.Conclusion:3.0T MR diffusion tensor imaging can reflect the diffusion characteristics of white matter water molecules in temporal lobe of nasopharyngeal carcinoma after radiotherapy, understand the regularity of its repair process, provide theoretical basis for clinical practice, and help to monitor the degree of radiation damage.
