摘要
目的对我院静脉用药集中调配中心成品输液在病房出现的质量问题进行分析和干预,提高输液质量。方法对我院静脉用药集中调配中心2016年-2017年的成品输液在病房出现的质量问题进行分类统计,并对2017年实施干预后的效果进行跟踪。结果 2016年成品输液在病房出现375例次质量问题,发生率为0.1356‰。主要表现为输液漏液、输液遗失、配置时间不当、配置不合理医嘱、配置错误、病人病情变化导致药品久置、出现絮状物或可见性微粒或变色以及护士操作不当等类型;自2017年1月起采取加强配置操作标准化、加强处方审核、加强药品管理以及加强对临床护士的用药培训等干预措施后,2017年成品输液在病房出现169例次质量问题,发生率为0.0922‰,较2016年明显下降。结论采取的干预措施有效降低了成品输液在病房出现质量问题的发生率,有助于提高输液质量,保证用药安全。
Objective To analyze the ward-appeared quality issues of finished infusions of Pharmacy Intravenous Admixture Services(PIVAS), in order to improve the quality of infusions. Methods The quality issues of finished infusions, which were found in the wards in2016 and 2017, were classified, calculated and statistically analyzed. The effects of intervention were discussed. Results There were 375 cases of quality issues of finished infusion in the wards in 2016, and the incidence was 0.1356‰. The causes mainly included leakage or loss of finished infusion, inopportune time of configuration, irrational medical order, configuration errors, long storage time, floe objects or visible particles or discoloration, improper operation by clinical nurse and so on. Since January 2017, intervention measures were taken such as enforcing the standardization of allocation operations, intensifying prescription audit, reinforcing drug management, and strengthening training for clinical nurses. Consequently, in 2017, only 169 cases of quality issues of finished infusion occurred in the wards, and the incidence rate was 0.0922‰. It was significantly lower in 2017 than in 2016. Conclusion The intervention measures can effectively reduce the incidence of quality problems of finished infusions in the wards. They contribute to the improvement of quality of infusions and the safety of drug use.
