摘要
目的探讨冠心病患者外周血微小RNA-126(miR-126)与血管间粘附分子-1(s VCAM-1)、相关炎性因子的表达及相关性,分析其在冠心病发生、发展中可能所起的作用。方法前瞻性选取冀中能源邢台矿业集团有限责任公司总医院住院确诊的冠心病患者80例为研究组,急性心肌梗死23例,不稳定性心绞痛38例,稳定性心绞痛19例;冠状动脉受累单支23例,多支57例。另选取健康40例中老年人为对照组。采用实时荧光PCR检测外周血清miR-126,夹心酶联免疫吸附法检测s VCAM-1,免疫透射比浊法测定超敏C反应蛋白(hs-CRP),双抗夹心法测定肿瘤坏死因子(TNF-α)、白细胞介素-1(IL-1)、白细胞介素-6(IL-6)水平。比较不同类型、不同冠状动脉受累程度冠心病患者血清miR-126、s VCAM-1及相关炎性因子水平。分析冠心病组患者血清miR-126水平与s VCAM-1及相关炎性因子相关性。结果冠心病组miR-126表达量低于对照组(P <0. 05),s VCAM-1及相关炎性因子表达水平高于对照组(P <0. 05)。急性心肌梗死患者miR-126表达量低于不稳定性和稳定性心绞痛者(P <0. 05),急性心肌梗死患者s VCAM-1及炎性因子IL-1、IL-6、TNF-α、hs-CRP表达均高于不稳定和稳定性心绞痛组(P <0. 05)。多支病变患者miR-126表达量低于单支病变(P <0. 05),s VCAM-1及炎性因子IL-1、IL-6、TNF-α、hs-CRP表达量高于单支病变(P <0. 05)。Pearson相关分析冠心病组患者血miR-126与s VCAM-1及炎性因子IL-1、IL-6、TNF-α、hs-CRP表达均呈负相关(P <0. 05)。结论冠心病患者存在明显的miR-126表达下调,s VCAM-1及相关炎性因子表达上调,与病情活动及冠状动脉受累严重程度密切相关,下调的miR-126失去对VCAM-1及相关炎性因子的抑制可能是导致冠心病发生及发展的主要机制之一。
Objective To investigate the expression and correlation of microRNA-126(miR-126)with vascular adhesion molecule-1(s VCAM-1)and inflammatory cytokines in peripheral blood of coronary heart disease(CHD)patients,and to analyze its possible mechanism.Methods 80 patients diagnosed with CHD and treated in the General Hospital of Suizhong Energy Xingtai Mining Group Co.,Ltd.were prospectively enrolled as the study group(n=80),including 23 patients with acute myocardial infarction,38 patients with unstable angina pectoris,19 patients with stable angina pectoris;23 cases were with single branch involvement,57 cases were with multi-branch involvement.Another 40 healthy middle-aged and elderly people were selected as the control group(n=40).Plasma miR-126 levels were detected with qRT-PCR and s VCAM-1 was detected by ELISA.TNF-α,interleukin-1(IL-1)and interleukin-6(IL-6)were measured by double-antibody sandwich method.Serum levels of miR-126,s VCAM-1 and related inflammatory factors were compared and their correlations analyzed for different CHD types and numbers of involved coronary arteries.Results The expression of miR-126 in CHD group was lower than in control group(P<0.05);the expression level of s VCAM-1 and related inflammatory factors was higher in CHD group than in control group(P<0.05).The expression of miR-126 in acute myocardial infarction patients was lower than in unstable and stable angina patients(P<0.05).s VCAM-1 and inflammatory factors IL-1,IL-6,TNF-α,hs-CRP in acute myocardial infarction patients than in unstable and stable angina patients(P<0.05).The expression of miR-126 in patients with multivessel disease was lower than in single-vessel disease(P<0.05),and the expression of s VCAM-1 and inflammatory factors IL-1,IL-6,TNF-αand hs-CRP were higher than in single-vessel disease(P<0.05).Pearson correlation analysis showed that there was a negative correlation of blood miR-126 with s VCAM-1 and inflammatory factors IL-1,IL-6,TNF-αand hs-CRP expression in patients with CHD(P<0.05).Conclusion CHD patients tend to have lower miR-126 which lead to upregulation of s VCAM-1 and inflammatory factors,all closely related to severity of the disease.Decreased mir-126 will lead to poor inhibited VCAM-1 and inflammation reaction,which may be one of the main mechanisms in CHD occurrence and development.
