摘要
目的探讨慢性踝关节不稳定患者行走时踝关节在体运动学的变化。方法我院接诊的48例慢性踝关节不稳定患者(观察组),选取同期体检的踝关节正常人群32例(对照组),观察两组踝关节运动角度和位移情况。结果胫骨-距骨和胫骨-跟骨:观察组足跟着地到站立中期前移距离、内旋角度、内翻角度较对照组增大,站立中期到足趾离地内旋角度较对照组增大(P <0. 05);距骨-跟骨:两组骨与骨之间运动距离和运动角度比较差异无统计学意义(P> 0. 05);两组三维平面总活动度比较差异无统计学意义(P> 0. 05),观察组跟骨着地后踝关节前5%~后15%(外翻-内翻)程度低于对照组(P <0. 05)。结论慢性踝关节不稳定患者行走时踝关节在体运动学发生明显变化,足跟着地前后踝关节前移内旋内翻程度加深,极易引发患者软骨损伤、骨关节炎等疾病。
Objective To investigate the changes in the kinematics of the walking ankle joint in patients with chronic ankle instability.Methods Forty-eight patients with chronic ankle instability who were admitted to our hospital were selected as study subjects(observation group).Thirty-two healthy people with normal ankle joints were selected as control group.The ankle joint movement angle and displacement were observed in both groups.Results For tibia-talus and humerus-calcaneus,the moving forward distance from heelstrike to middle of standing,internal rotation angle,and inversion angle as well as the internal rotation angle from middle of standing to toe was off the ground in the observation group were larger than the control group(P<0.05).For talus-calcane,there was no significant difference in the moving distance and angle between the bones when compare the two groups(P>0.05).There was also no significant difference in the three-dimensional total plane activity between the two groups(P>0.05).The degree of anterior 5%~post 15%(valgus and varus)of the ankle joint after the heelstrike in the observation group was lower than that of the control group(P<0.05).Conclusion In patients with chronic ankle instability,the ankle joints undergo significant changes in body kinematics.The degree of internal rotation and inversion before and after heel strike are deepened.Thus.it is easy to cause cartilage damage,osteoarthritis and other diseases.
