摘要
目的观察缺血预适应对脑梗死患者血清凋亡相关因子的影响。方法 84例急性脑梗死患者根据脑梗死前出现短暂性脑缺血发作与否,分为对照组与观察组,观察组根据短暂性缺血持续时间分为A组(≤10 min,15例)、B组(10~20min,11例)、C组(21~60 min,10例)、D组(60分钟以上,8例)。5组均给予相同对症支持治疗。比较治疗后梗死面积、神经功能、预后及血清凋亡相关因子水平。结果 5组治疗后梗死灶面积小于治疗前,血清B淋巴细胞瘤-2(Bcl-2)水平高于治疗前,NIHSS评分及细胞色素C(Cyt)、P53、白介素-2(IL-2)水平低于治疗前(P <0. 05)。B组治疗后梗死灶面积小于对照组,Ⅰ级预后比例及Bcl-2水平高于对照组,NIHSS评分及Cyt、P53、IL-2水平低于对照组(P <0. 05)。结论 10~20 min的缺血预适应可显著缩小急性脑梗死患者梗死灶面积,改善神经功能缺损,提高预后,可能与提高血清Bcl-2水平,降低血清Cyt、P53、IL-2水平有关。
Objective To observe the effect of ischemic preconditioning on serum apoptosis related factors in patients with cerebral infarction.Methods Eighty-four patients with acute cerebral infarction were divided into control group and observation group according to whether they had transient ischemic attack before cerebral infarction. According to the duration of transient ischemia,the observation group was further divided into group A( no more than 10 min,n = 15),group B( 10 ~ 20 min,n = 11),group C( 21 ~ 60 min,n = 10) and group D( more than 60 minutes,n = 8).All 5 groups were given the same symptomatic support treatment.The infarct area,neurological function,prognosis and serum apoptosis related factors were compared after treatment. Results The infarct areas of the 5 groups were significantly decreased,serum Bcl-2 levels were increased,NIHSS score and levels of Cyt,P53 and IL-2 were decreased after treatment when compared to before treatment( P < 0. 05).After treatment,the infarct areas were significantly smaller,proportion of grade Ⅰ prognosis and serum Bcl-2 levels were higher and NIHSS score and levels of Cyt,P53 and IL-2 were lower in the group B than those in the control group( P < 0. 05).Conclusion 10 ~ 20 min of ischemic preconditioning can significantly narrow the infarct area of patients with acute cerebral infarction,improve the neural function defects and improve the prognosis.These may be related to improving serum bcl-2 level and reducing serum levels of Cyt,P53,and Il-2.
