摘要
目的探讨联合4种不同的血清学指标检测,应用于早期诊断卵巢恶性病变中的价值。方法随机选取2015年6月至2017年6月南部战区海军第二医院诊治的100例卵巢恶性病变患者,将100例卵巢癌患者设置为卵巢恶性病变组;随机抽取同时期共23例交界性癌患者患者,将上述人群设置为交界性肿瘤组;随机抽取同时期卵巢良性病变患者100例,设为良性病变组。将三组患者作为研究对象,对其临床资料进行分析。检测血清间皮素、HE4、VEGF-C、CA125水平,比较分析联合应用4种血清学指标对卵巢恶性病变的诊断效果。结果卵巢恶性病变患者与交界性肿瘤患者血清间皮素、HE4、VEGF-C、CA125平均值均高于良性病变组,差异具有统计学意义(P<0.05),但卵巢恶性病变患者与交界性癌患者之间差异无统计学意义(P>0.05);随着恶性程度增加,卵巢恶性病变患者上述4种血清学指标平均值均升高,差异具有统计学意义(P<0.05);对卵巢恶性病变的诊断效果,单独检测HE4比单独检测另外3种血清学指标更好,血清间皮素、VEGF-C、CA125、HE4同时进行检测,诊断卵巢恶性病变的灵敏度与特异度最高,差异具有统计学意义(P<0.05)。结论血清间皮素、VEGF-C、CA125、HE4联合检测对卵巢恶性病变的诊断效能较好,在病情监测中也具有一定意义。
ObjectiveTo investigate the value of four different serological markers in early diagnosis of ovarian cancer. MethodsA total of 100 patients with ovarian cancer were randomly selected from April 2015 to August 2017 in the 425 Hospital of the People′s Liberation Army as malignant lesions group. A total of 23 patients with borderline cancer were randomly selected as the borderline cancer group. Another 100 patients with benign ovarian lesions at the same time were randomly selected as benign lesions group. The serum levels ofmesothelin, HE4, VEGF-C and CA125 in all the subjects were detected. The diagnostic effects of the four serological markers on ovarian malignant lesions were compared and analyzed. ResultsThe mean serum levels ofmesothelin, VEGF-C, HE4 and CA125 in patients in malignant lesions group and borderline cancer group were higher than those in benign lesions group, with statistically significant differences (P< 0.05), but the difference between malignant lesions group and borderline cancer group was not statistically significant (P> 0.05). With the increase of malignancy, the average levels of the above four markers in patientswith ovarian cancer increased, with statistically significant difference (P< 0.05). The sensitivity and specificity of the diagnosis of ovarian malignant lesions was the highest when the serum levels of mesothelin, VEGF-C, CA125 and HE4 were detected jointly, with statistically significant difference (P> 0.05). ConclusionsThe combined detection of serum levels of mesothelin, VEGF-C, CA125 and HE4 is of good efficacy in the diagnosis of ovarian malignant diseases, which also has some significance in the monitoring of the disease.
