摘要
目的探讨磁共振成像技术(MRI)诊断腔隙性脑梗死患者的临床价值。方法选取100例腔隙性脑梗死患者作为研究对象,对其进行头颅MRI、计算机断层扫描(CT)检查,比较MRI、CT检出率以及检出的病灶部位、大小差异。结果①本组患者的腔隙性脑梗死病灶共149个:MRI检出147个,其诊断准确度是98. 6%(147/149),敏感度是98. 6%(147/149),特异度是0; CT检出病灶125个,其诊断准确度是83. 9%(125/149),敏感度是83. 9%(125/149),特异度是0,MRI对本组腔隙性脑梗死病灶的诊断准确度、敏感度高于CT,差异有统计学意义(P <0. 05);②MRI对直径≤5 mm的病灶检测准确率是100. 0%,高于CT的86. 8%(χ2=12. 847,P <0. 001),对直径> 5 mm的病灶检测准确率是96. 6%,高于CT的79. 3%(χ2=8. 123,P=0. 004);③MRI对本组患者腔隙性脑梗死病灶部位的检测准确率均高于CT,但其检测准确率与CT差异无统计学意义(P> 0. 05)。结论 MRI诊断腔隙性脑梗死患者的临床价值显著,检出率较高,特别是对直径不足5 mm小病灶的检出率,远高于CT。
Objective To investigate the clinical value of magnetic resonance imaging (MRI) in the diagnosis of lacunar infarction.Methods A total of 100 patients with lacunar cerebral infarction were selected.The detection rates of MRI,CT and the location and size of the lesions were compared.Results There were 149 lesions of lacunar infarction enrolled:147 lesions were detected by MRI,the diagnostic accuracy was 98.6%(147/149),the sensitivity was 98.6%(147/149),the specificity was 0;and 125 lesions were detected by CT,the diagnostic accuracy was 83.9%(125/149),the sensitivity was 83.9%(125/149),the specificity was 0.The diagnostic accuracy and sensitivity of MRI for lacunar infarction in this group were higher than those of CT.The difference of CT was statistically significant (P<0.05).The detection accuracy of lesions with diameter less than 5 mm by MRI was 100.0%,higher than that by CT (86.8%),χ^2=12.847,P<0.001.The detection accuracy of lesions with diameter greater than 5 mm was 96.6%,higher than that by CT (79.3%),χ^2=8.123,P=0.004.The detection accuracy of lacunar infarction lesions by MRI was higher than that by CT (χ^2=8.123,P=0.004).The accuracy of MRI in detecting lesions of lacunar infarction in this group was higher than that of CT,but there was no significant difference between the accuracy of MRI and CT (P>0.05).Conclusion The clinical value of magnetic resonance imaging in the diagnosis of lacunar infarction is remarkable,and the detection rate is high,especially for small lesions less than 5mm in diameter,which is much higher than that of CT.
