摘要
目的:观察十全大补汤改善肺癌患者化疗后癌因性疲乏(CRF)的效果及作用机制。方法:按随机数表法将60例肺癌化疗后患者分为十全大补汤组(研究组,n=30)和常规组(对照组,n=30)。于治疗前(T1)、治疗2周后(T2)评估两组患者CRF改善情况[修订版Piper疲乏量表(RPFS)]和疾病情况[卡氏评分(KPS)、中医症状评分(原发性肺癌症状分级量表)],比较两组患者T1、T2时血常规[白细胞(WBC)、血红蛋白(Hb)、中性粒细胞(Neu)]、T淋巴细胞亚群(CD_4^+、CD_8^+、CD_3^+)、皮质醇(Cor)及甲状腺功能相关指标[促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)]水平差异。结果:T2时,两组KPS评分和血WBC、Hb、CD_4^+、CD_3^+水平、血清FT3水平均较T1时升高,且研究组高于对照组(P<0.05);原发性肺癌症状分级量表和RPFS评分、血CD8+水平及血清Cor、TSH水平则均较T1时降低,且研究组低于对照组(P<0.05)。两组血Neu水平均较T1时升高(P<0.05),但组间比较差异无统计学意义(P>0.05)。结论:十全大补汤可有效改善肺癌化疗患者健康状况、中医症状,减轻癌症及化疗对患者血常规、免疫功能、内分泌功能等方面的影响,对改善患者化疗后CRF有一定帮助。
Objective:To observe the effects and role mechanism of Shiquan Dabu Decoction on improving cancer-induced fatigue(CRF)in patients with lung cancer after chemotherapy. Methods:60 cases of patients with lung cancer after chemotherapy were divided into Shiquan Dabu Decoction group(study group,n=30)and routine group(control group,n=30)according to the random number table method.The CRF improvement [revised piper fatigue scale(RPFS)] and disease status [karnofsky performance status(KPS),TCM symptoms scores(primary lung cancer symptom rating scale)] were assessed in the two groups before treatment(T1)and after 2 weeks of treatment(T2),and the levels of blood routine[white blood cell(WBC),hemoglobin(Hb),neutrophil(Neu)],T lymphocyte subsets(CD4^+,CD8^+,CD3^+),cortisol(Cor)and related indexes of thyroid function [thyroid stimulating hormone(TSH),free triiodothyronine(FT3)]were compared between the two groups at T1 and T2. Results At T2,the KPS score and levels of blood WBC,Hb,CD4^+ and CD3^+ and serum FT3 in the two groups were higher than those at T1,and the indexes in study group were higher than those in control group(P<0.05). The scores of primary lung cancer symptom rating scale and RPFS,and the levels of CD8^+ and serum Cor and TSH were all lower than those at T1,and the indexes in study group were lower than those in control group(P<0.05). The level of blood Neu in the two groups was higher than that at T1(P<0.05),but there was no significant difference between the two groups(P>0.05). Conclusions:Shiquan Dabu Decoction can effectively improve the health status and TCM symptoms of patients with lung cancer chemotherapy,reduce the impact of cancer and chemotherapy on the blood routine,immune function and endocrine function,and it has certain help to improve the CRF after chemotherapy.
