摘要
目的:观察比较中药外洗与银离子泡沫敷料治疗糖尿病足溃疡(湿热毒盛证)的临床疗效。方法:将94例糖尿病足患者随机分为治疗组和对照组各47例,在常规治疗的基础上,治疗组采用中药熏洗,对照组患者用银离子泡沫敷料外敷,疗程均为4周,并进行6~24个月的随访。观察比较两组患者治疗期间每周的创面愈合指数、临床疗效、安全性及复发率。结果:治疗组第1、2、3、4周的创面愈合指数均较对照组高,差异有统计学意义(均P<0.05);治疗组的疗效优于对照组,差异有统计学意义(P<0.05);两组患者均无明显不良反应发生;复发率治疗组7.3%,低于对照组28.2%,差异有统计学意义(P<0.05)。结论:银离子敷料和中药外洗对糖尿病足溃疡均具有较好疗效,但中药外洗在促进伤口愈合、降低感染复发率更具有优势,且价格低廉,值得临床推广。
Objective: to observe the clinical efficacy of chinese herbal fumigation and washing by comparing it with silver ion foam dressing in the treatment of diabetic foot ulcer (Damp-heat syndrome). Methods: Ninety-four patients with diabetic foot were randomly divided into the treatment group and the control group, with 47 cases in each group. On the basis of routine treatment, the treatment group was treated with Chinese herbal fumigation and washing, at the same time the control group was treated with silver ion foam dressing for 4 weeks. the following-up was for 6-24 months. the weekly wound healing index, clinical efficacy, safety and recurrence rate were compared between the two groups during treatment. Results: The wound healing index at the 1st, 2nd, 3rd, and 4th week in the treatment group was higher than that in the control group (P < 0.05). The curative effect of the treatment group was better than that of the control group, and the difference was statistically significant (P < 0.05);There was no obvious adverse reaction in both groups;the recurrence rate was 7.3% in the treatment group and 28.2% in the control group, which was of significant difference (P < 0.05). Conclusion: Silver ion dressing and external Chinese herbal fumigation and washing both had good curative effects on diabetic foot ulcers, but Chinese herbal fumigation and washing had advantages in promoting wound healing and reducing the recurrence rate of infection, and it was inexpensive and was worthy of clinical promotion.
