摘要
目的观察甲状腺功能亢进(甲亢)患者治疗前后体质量、脂联素水平以及血糖代谢水平的变化及其相关性,为甲亢患者的临床诊治提供参考。方法选择2015年1月—2017年1月海南省人民医院内分泌科门诊治疗甲亢患者60例作为研究对象(甲亢组),均给予甲硫咪唑和丙硫氧嘧啶药物治疗,记录患者治疗前后的体质量变化、血清脂联素(APN)、甲状腺功能指标、血糖以及血脂代谢相关指标。并选择同期体检的34例健康人作为健康对照组,分别于治疗前、治疗后6个月进行随访,观察甲亢患者治疗前后的体质量变化情况,分析血清脂连素水平与血糖代谢水平间的相关性。结果与治疗前比较,治疗后甲亢患者的体质量和血清TSH、HDL-C水平升高(t/P=11.871/<0.001、19.741/<0.001、24. 316/<0.001),血清FT_3、FT_4、APN、FPG、2 hPG、TC以及LDL-C水平降低(t/P=23.676/<0.001、27.521/<0.001、10.787/<0.001、12. 068/<0. 001、7. 116/<0. 001、22. 264/<0. 001、8. 210/<0. 001)。与健康对照组比较,甲亢组患者治疗后的体质量、甲状腺、脂联素、血糖代谢指标以及血脂水平无明显变化(P>0. 05)。甲亢组患者血清脂联素与血清FPG、2 hPG水平呈现正相关(r/P=0. 374/0. 023、0. 869/<0. 01)。治疗后,54例甲亢患者体质量增加至治疗前水平,6例患者体质量超标,体质量变化曲线大概呈现"V"型,变化规律与治疗过程中血清FT_4、FT_3水平下降趋势相一致。结论甲亢患者治疗前后的体质量呈现"V"形变化,且治疗后患者的血脂、血糖代谢水平改善显著,血清脂联素水平与血糖代谢水平呈现正相关,具有一定的临床应用价值。
Objective To analyze the changes of body mass, adiponectin level and blood glucose metabolism before and after treatment in patients with hyperthyroidism and their correlation, and provide reference for the clinical diagnosis and treatment of patients with hyperthyroidism. Methods From January 2015 to January 2017, 60 patients with hyperthyroidism in the Department of Endocrinology, Hainan Provincial People’s Hospital were selected as subjects. All patients were treated with propylthiouracil and methimazole, and the changes of body weight, serum adiponectin( APN), thyroid function index,blood glucose and blood lipid metabolism were recorded before and after treatment. Thirty-four healthy people who came to our hospital for examination in the same period were selected as the control group. Follow-up was conducted before treatment, 12 months after treatment and 24 months after treatment, respectively, to observe the body mass changes of hyperthyroidism patients before and after treatment, and to analyze the correlation between serum adiponectin level and glucose metabolism level.Results Compared with before treatment, the body weight and serum TSH and HDL-C levels of patients with hyperthyroidism increased after treatment(t/P = 11.871/< 0. 001, 19. 741/< 0. 001, 24. 316/< 0. 001), serum FT3,FT4, APN, FPG,2 hPG, TC, and LDL-C levels were reduced(t/P = 23. 676/< 0. 001,27. 521/< 0. 001,10. 787/< 0. 001, 12. 068/<0.001,7.116/<0.001,22.264/<0.001,8.210/<0.001). Compared with the control group,the body weight,thyroid,adiponectin, blood glucose metabolism index and blood lipid level of patients with hyperthyroidism did not change significantly after treatment(P >0. 05). In hyperthyroidism group,the serum adiponectin was positively correlated with serum FPG,2 hPG(r/P =0.374/0.023,0. 869/<0. 01) Conclusion The body weight of patients with hyperthyroidism showed a "V" shape change before and after treatment, and the blood lipid and blood glucose metabolism levels of the patients after treatment were significantly improved. The serum adiponectin level was positively correlated with the blood glucose metabolism level and had certain clinical application value.
