摘要
目的探讨疏肝汤联合米氮平对肝郁化火型失眠患者的临床疗效。方法 118例患者随机分为对照组(56例)和观察组(62例),对照组给予米氮平,观察组在对照组基础上加用疏肝汤,8周为1个疗程。然后,检测临床疗效,中医症候评分,下丘脑-垂体-靶腺轴激素(TSH、TRH、GH、Cor、FT4)水平,匹兹堡睡眠质量指数量表(PSQI)评分,焦虑、抑郁评分,生活质量评分,不良反应发生率变化。结果观察组总有效率显著高于对照组(P<0.05)。治疗后,2组中医症候评分,下丘脑-垂体-靶腺轴激素水平,PSQI评分,焦虑、抑郁评分显著降低(P<0.05),生活质量评分显著升高(P<0.05),以观察组更明显(P<0.05),不良反应发生率无显著差异(P>0.05)。结论疏肝汤联合米氮平可调节肝郁化火型失眠患者下丘脑-垂体-靶腺轴激素表达,改善睡眠质量和生活质量,安全性较高,临床疗效良好。
AIM To explore the clinical effects of Shugan Decoction combined with mirtazapine on insomnia patients due to Liver Depression Transforming into Fire.METHODS One hundred and eighteen patients were randomly divided into control group(56 cases)for 8-week administration of mirtazapine,and observation group(62 cases)for 8-week administration of Shugan Decoction and mirtazapine.Subsequently,the changes in clinical efficacy,TCM symptom score,levels of hypothalamus-pituitary-target gland axis hormones(TSH,TRH,GH,Cor,FT4),Pittsburgh sleep quality index scale(PSQI)score,anxiety and depression scores,life quality score,and occurrance rate of adverse reactions were detected.RESULTS The observation group demonstrated significantly higher total effective rate than the control group(P<0.05).After the treatment,TCM syndrome score,levels of hypothalamic-pituitary-target gland axis hormones,PSQI score,and anxiety and depression scores were markedly decreased(P<0.05)in the two groups with obviously increased life quality score(P<0.05),especially for those in the observation group(P<0.05),and no significant difference in occurrance rate of adverse reactions was found(P>0.05).CONCLUSION For the insomnia patients due to Liver Depression Transforming into Fire,Shugan Decoction combined with mirtazapine can regulate the expressions of hypothalamus-pituitary-target gland axis hormones and improve sleep quality and life quality,with high safety and good clinical efficacy.
