摘要
目的探讨抗生素骨水泥型假体旷置治疗髋关节置换术后假体周围感染的临床效果。方法回顾性分析2015年1月至2018年1月郑州大学第一附属医院收治的17例髋关节置换术后假体周围慢性感染患者的临床资料,所有患者接受抗生素骨水泥型假体旷置治疗。统计患者术后2周切口愈合情况。采用Harris评分比较患者术前2周和术后2个月髋关节功能。采用视觉模拟评分法(VAS)比较术前2周和术后2个月患者的疼痛程度。比较术前2周和术后2个月患者的红细胞沉降率(ESR)、C反应蛋白(CRP)。结果术后2周拆除切口缝线,17例患者切口愈合良好,无红肿、渗出。术后2个月Harris评分高于术前2周,术后2个月VAS评分、CRP、ESR均低于术前2周,差异有统计学意义(均P<0.05)。结论利用抗生素骨水泥型假体旷置治疗髋关节置换术后假体周围感染的效果确切,能更好地控制感染,保留患髋关节功能,减轻患者疼痛。
Objective To explore the effect of antibiotic-loaded bone cement prosthesis exclusion on periprosthetic infection after hip replacement. Methods A retrospective analysis of 17 patients with chronic infection around the prosthesis after hip replacement between January of 2015 and January of 2018 was conducted. All patients received the therapy of antibiotic-loaded bone cement prosthesis exclusion. The healing of incision 2 weeks after operation was counted. Harris score was used to compare the function of hip joint 2 weeks before operation and 2 months after operation. Visual analogue scale (VAS) was used to compare the degree of pain 2 weeks before operation and 2 months after operation. The erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) of patients 2 weeks before operation and 2 months after operation were compared. Results The sutures of incision were removed 2 weeks after the operation. The incision healed well in 17 patients without red swelling and exudation. Harris score was higher 2 months after operation than 2 weeks before operation, and VAS score, CRP, ESR were lower 2 months after operation than 2 weeks before operation, and the differences were statistically significant (all P < 0.05). Conclusion The use of antibiotic-loaded bone cement prosthesis for the treatment of periprosthetic infection after hip replacement has definite effect, which could better control infection, retain the function of the affected hip joint and relieve the pain of the patient.
