摘要
目的研究耳内镜下鼓膜置管术对分泌性中耳炎的治疗效果,分析其作用。方法选择2017年5月—2018年10月于我院鼓膜置管术患者77例,给予耳内镜下鼓膜置管术治疗并采用纯音测听及声导抗检查判断临床效果。结果 77例患者鼓膜置管术均成功,鼓膜和外耳道未见损伤。随访1周发现4例哑铃形管脱垂,鼓膜愈合,分泌性中耳炎复发,再行置管术;1例哑铃形管拔管后鼓膜穿孔未愈合,72例拔管后4周左右痊愈。术后1个月采用纯音测听及声导抗检查发现77例患者中60例治愈、16例有效、1例低效,总治愈率98.7%。结论针对分泌性中耳炎应用耳内镜下鼓膜置管术治疗,可以清晰观察鼓膜的环境,从而提高手术的成功率,创伤小,且有效治愈患者的听力功能。
Objective To study the therapeutic effect of endoscopic tympanic membrane occlusion on secretory otitis media and analyze its effect. Methods 77 patients with tympanic membrane catheterization from May 2017 to October 2018 were enrolled in this study. The patients underwent endoscopic tympanic catheterization and were judged by pure tone audiometry and acoustic impedance test. Results In 77 patients, tympanic membrane placement was successful, and no obvious damage was found in the tympanic membrane and external auditory canal. After 1 week of follow-up, 4 cases of dumbbell-shaped tube prolapse, tympanic membrane healing, recurrent otitis media recurrence, and catheterization were performed. In 1 case, the tympanic membrane perforation was not healed after extubation of the dumbbell-shaped tube, and 72 cases were cured about 4 weeks after extubation. One month after operation, pure tone audiometry and acoustic impedance test showed that 60 of 77 patients were cured, 16 patients were effective, and 1 patient was ineffective. The total cure rate was 98.7%. Conclusion The application of endoscopic tympanic membrane catheterization for secretory otitis media can clearly observe the tympanic membrane environment, thus improving the success rate of surgery, less trauma, and effectively curing the hearing function of patients.
