摘要 目的探讨高压氧治疗在改善重度颅脑损伤手术后患者神经功能及肢体功能中的作用。方法将2015年8月~2018年5月于我院手术的重度颅脑损伤患者112例随机分为常规组(术后行常规治疗干预)和高压氧组(术后行常规治疗+高压氧干预)各56例。分析两组干预效果、神经功能和肢体功能相关指标及评分差异。结果高压氧组干预总有效率(85.71%)高于常规组(66.07%)(P <0.05);干预后两组血清NSE、BDNF和GFAP等神经功能指标低于干预前,GCS昏迷评分高于干预前,且高压氧组各指标均优于常规组(P <0.05);干预后两组Fugl-Meyer上、下肢功能评分和总分均高于干预前,且高压氧组分值高于常规组(P <0.05)。结论高压氧治疗能够显著改善重度颅脑损伤患者术后神经功能及肢体功能等指标,促进神经和运动功能恢复。 Objective To explore the role of hyperbaric oxygen therapy in improving neurological function and limb function of patients with severe craniocerebral injury after operation. Methods 112 patients with severe craniocerebral injury who were operated in our hospital from August 2015 to May 2018 were randomly divided into routine group(routine treatment intervention after operation) and hyperbaric oxygen group(postoperative routine treatment + hyperbaric oxygen intervention),each had 56 cases.The intervention effect,neurological function and limb function related indicators and score differences between the two groups were analyzed. Results The total effective rate of hyperbaric oxygenation group(85.71%) was higher than the routine group(66.07%)(P < 0.05).After intervention,the indexes of serum NSE, BDNF and GFAP of the two groups were lower than before intervention, and GCS coma score was higher than before intervention,and the indexes of hyperbaric oxygen group were all better than the routine group(P < 0.05).After intervention,the upper and lower extremity function scores and total scores of Fugl-Meyer in the two groups were higher than before intervention,and the values of hyperbaric oxygen group were higher than routine group(P < 0.05). Conclusion Hyperbaric oxygen therapy can significantly improve postoperative neurological function and limb function of patients with severe craniocerebral injury, and promote the recovery of nerve and motor function.
