应用LIS大数据建立0~3岁儿童血清FT3、FT4、TSH间接参考区间 被引量:2

Establishment of serum FT3,FT4 and TSH reference intervals for children of 0-3 years by laboratory information system

摘要 目的应用实验室信息系统(LIS)大数据建立0~3岁儿童血清甲状腺功能指标——游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)间接参考区间。方法收集2013年7月至2017年12月该院检验科LIS中FT3、FT4、TSH检测值29 169例,受检者年龄1d~3岁,分为A组(1d~≤1个月)、B组(>1~≤12个月)、C组(>1~≤3岁),经数据筛选,结合R语言编程,线性拟合,建立间接参考区间。结果 A组受检者FT3 2.12~7.42pmol/L,FT4 10.87~31.20pmol/L,TSH 0.681~18.673μIU/mL;B组受检者FT3 4.34~7.61pmol/L,FT4 10.90~20.14pmol/L,TSH 0.388~5.401μIU/mL;C组受检者FT3 4.05~7.41pmol/L,FT4 11.28~20.50pmol/L,TSH 0.279~5.139μIU/mL。利用LIS存储数据建立的0~3岁3个年龄段儿童FT3、FT4、TSH参考区间与文献报道基于健康个体建立的参考区间比较,差异均无统计学意义(P>0.05)。结论应用LIS大数据建立检验项目的参考区间经济、简便、可行,可作为实验室自建参考区间的一种选择。 Objective To establish the reference intervals of free Triiodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH)for children of 0-3 years children by laboratory information system (LIS) from hospital-stored data in Renmin Hospital of Wuhan University.Methods A total of 29 169 cases with the age of 0-3 years of the test results of serum FT3,FT4 and TSH stored in the LIS of Renmin Hospital of Wuhan University from July 2013 to December 2017 were collected and divided into group A( 1 d-≤ 1 month),group B(>1-≤12 months)and group C(>1-≤3 years),and after data screening,R-language programming and linear fitting were used to establish the indirect reference intervals.Results The reference intervals of group A were:FT3 2.12-7.42 pmol/L,FT4 10.87-31.20 pmol/L and TSH 0.681- 18.673 μIU/mL,the group B were FT3 4.34-7.61 pmol/L,FT4 10.90-20.14 pmol/L and TSH 0.388- 5.401 μIU/mL;the group C were FT3 4.05-7.41 pmol/L,FT4 11.28-20.50 pmol/L,TSH 0.279- 5.139 μIU/mL.There was no statistically significant difference (P>0.05) on the FT3,FT4,TSH reference intervals between the existing reference ranges established by healthy individuals and using LIS storage data.Conclusion Application of LIS to establish the reference intervals is economy,simple and feasible,which can be used as a choice of the reference interval of the laboratory.
