摘要
目的探究核转运蛋白基因2(KPNA2)表达与上尿路尿路上皮癌(UTUC)患者临床病理参数和预后之间的关系。方法采用组织芯片结合免疫组化染色法检测KPNA2在265例上尿路尿路上皮癌组织和146例癌旁正常上尿路上皮组织中的蛋白表达水平,分析其表达水平与患者年龄、性别、T分期、G分级、肿瘤大小、肿瘤边侧和术后膀胱复发等临床病理参数之间的关系,探讨KPNA2对患者预后的影响。结果 KPNA2在上尿路尿路上皮癌组织中的高表达率(74.34%)明显高于其对应的癌旁组织中的高表达率(24.66%)(P<0.05)。KPNA2蛋白表达水平与患者的T分期(P=0.000)、G分级(P=0.033)和术后有无膀胱复发(P=0.004)有关,而与患者的年龄(P=0.265)、性别(P=0.312)、肿瘤大小(P=0.582)和肿瘤边侧(P=0.386)无关。术后随访12~108个月(平均36个月),共有56例出现膀胱复发,总复发率为21.1%。高表达KPNA2的患者5年总体生存率显著低于无或低表达KPNA2的患者(P=0.007 6)。结论 KPNA2在上尿路尿路上皮癌组织中高表达,其表达水平与上尿路尿路上皮癌的临床分期和病理分级密切相关,并影响患者预后。
Objective To explore the expression of karyopherin a2(KPNA2) and its relationship with the clinicopathological parameters and prognosis of upper tract urothelial carcinoma(UTUC).Methods The expression of KPNA2 was detected with tissue microarray and immunohistochemical staining in 265 UTUC cases and 146 normal paracancerous tissues.The relationships between KPNA2 expression and patients’ age and sex,T stage,G grade,tumor size,tumor side,bladder recurrence and prognosis were analyzed.Results The expression rate of KPNA2 was significantly higher in UTUC(74.34%) than in paracancerous tissues(24.66%,P<0.05).The protein expression of KPNA2 was positively correlated with T stage(P=0.000),G grade(P=0.033) and bladder recurrence(P=0.004),but was not associated with patients’ age(P=0.265),sex(P=0.312),tumor size(P=0.582) and tumor side(P=0.386).During the follow-up of 12 to 108(average 36) months,bladder recurrence occurred in 56 patients,with a total recurrence rate of 21.1%.The 5-year overall survival rate of patients with high expression of KPNA2 was significantly lower than that of patients without or with low expression of KPNA2(P=0.007 6).Conclusion KPNA2 is highly expressed in upper tract urothelial carcinoma,and its expression is closely related to the clinical stage,pathological grade and prognosis.
