剪切波结合病理对复杂背景下隐匿性甲状腺癌的弹性界值的实验研究 被引量:1

摘要 目的探讨剪切波结合病理对复杂背景下隐匿性甲状腺癌的弹性界值的实验研究。方法将105例(105枚)复杂背景下隐匿性甲状腺结节患者作为研究对象,对患者进行剪切波弹性成像技术检查,从而获取病灶VTIQ技术相关参数,并与病理诊断结果相结合。其中病理诊断良性结节60例,恶性结节45例。比较良恶性甲状腺结节病灶VTIQ技术相关参数的差异。同时采用受试者操作特性曲线(ROC)评价VTIQ技术诊断复杂背景下隐匿性甲状腺癌的价值。结果复杂背景下隐匿性甲状腺恶性结节VTIQ技术相关参数(最大值、最小值、平均值)显著高于复杂背景下甲状腺良性结节,具有统计学差异(P <0. 05);采用VTIQ技术弹性平均值及最大值绘制ROC曲线,AUC面积为0. 924、0. 943; VTIQ技术值最大值3. 9 m/s为界值时,诊断复杂背景下甲状腺良恶性结节的敏感性为90. 5%,特异性为86. 6%、准确性为85. 7%; VTIQ技术值平均值3. 6 m/s为界值时,诊断复杂背景下甲状腺良恶性结节的敏感性为84. 8%,特异性为87. 5%、准确性为83. 2%。结论剪切波对复杂背景下隐匿性甲状腺癌诊断价值较高,甲状腺恶性结节VTIQ技术弹性界值较高,在弹性最大值时诊断敏感性、特异性等均较高,值得临床选择。 Objective To investigate the elastic boundary value of shear wave combined with pathology in occult thyroid carcinoma under complex background. Methods 105 cases of occult thyroid nodules( 105) were treated as the subjects of occult thyroid nodules. The patients were examined by shear wave elastic imaging system,and the related parameters of Young’s modulus were obtained,and the results were combined with the pathological diagnosis. Among them,60 cases were benign nodules,45 cases were malignant nodules. The difference of Young’s modulus between benign and malignant thyroid nodules was compared. At the same time,we used the operator performance curve( ROC) to evaluate the value of Young’s modulus in diagnosing thyroid malignancy. Results The parameters related to Young’s modulus( maximum,minimum,average) of occult thyroid malignant nodules were significantly higher than those of occult thyroid benign nodules,and there were statistical differences. ROC curves were drawn with the elastic average and maximum of Young’s modulus,and the product of AUC was 0. 924 and 0. 943,and the young’s modulus was the most. The sensitivity of the diagnosis of thyroid benign and malignant nodules was 90. 5%,the specificity was 86. 6%,the accuracy was 85. 7%. When the mean value of Young’s modulus value was 3. 6 m/s as the boundary value,the sensitivity of the thyroid benign and malignant nodules was 84. 8%,the specificity was 87. 5%,and the accuracy was 83. 2%. Conclusion The shear wave was hidden under the complex background. The diagnostic value of sexual thyroid carcinoma is high,the young modulus of thyroid malignant nodules is higher than that of the young’s modulus,and the diagnostic sensitivity and specificity are high in the maximum value of elasticity,which is worthy of clinical selection.
