摘要
目的研究鼻内窥镜下低温等离子射频消融术(ELTPRA)治疗小儿鼾症伴过敏性鼻炎(AR)的效果。方法选择2017年6月～2018年6月在湖南省儿童医院接受治疗的鼾症伴AR患儿115例。按随机数字表法将患儿分成观察组(n=58)和对照组(n=57)。对照组患儿实施扁桃体联合腺样体切除术,观察组患儿实施ELTPRA。术后随访3个月,比较两组疗效、临床指标、治疗前及治疗3个月后的炎性因子,以及并发症情况。结果观察组总有效率是98.28%,明显高于对照组(89.47%),差异有统计学意义(P <0.05)。观察组的手术时间、术中出血量、住院时间均明显少于对照组,差异有高度统计学意义(P <0.01)。治疗后,两组白细胞介素-4(IL-4)、IL-8及肿瘤坏死因子-α(TNF-α)水平均有下降(P <0.05),观察组IL-4、IL-8及TNF-α水平均明显低于对照组,差异有高度统计学意义(P <0.01)。观察组的疼痛发生率、并发症的总发生率均低于对照组,差异有统计学意义(P <0.05)。结论 ELTPRA治疗鼾症伴AR患儿的效果较好,且并发症也较少,还可有效促进患儿的术后恢复,降低炎性因子水平,效果明显,值得推广。
Objective To study the effect of low temperature plasma radiofrequency ablation(ELTPRA)under nasal endoscope in the treatment of snoring with allergic rhinitis(AR)in children.Methods A total of 115 children with snoring and AR who were treated in Hu′nan Children′s Hospital from June 2017 to June 2018 were enrolled.The children were divided into observation group(n=58)and control group(n=57)according to the random number table method.The control group was treated with tonsillectomy combined with adenoidectomy,while the observation group was treated with ELTPRA.After 3 months follow-up,the curative effect,clinical indicators,inflammatory factors and complications before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 98.28%,which was significantly higher than that of the control group(89.47%),and the difference was statistically significant(P<0.05).The operation time,intraoperative bleeding volume and hospitalization time of the observation group were significantly less than those of the control group,and the differences were statistically significant(P<0.01).After treatment,the levels of interleukin-4(IL-4),IL-8 and tumor necrosis factor-alpha(TNF-α)were significantly decreased in both groups(P<0.05),and the levels of IL-4,IL-8,TNF-αin the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.01).The incidence of pain and complications in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion ELTPRA is effective in the treatment of children with snoring and AR,and has fewer complications.It can also effectively promote postoperative recovery and reduce the level of inflammatory factors in children,which is effective and worthy of promotion.
