摘要
目的观察电针联合膀胱综合训练对骶上脊髓损伤后神经源性膀胱(逼尿肌反射亢进型)患者膀胱功能的影响。方法采用随机数字表法将50例骶上脊髓损伤后神经源性膀胱(逼尿肌反射亢进型)患者分为对照组及观察组,每组25例。对照组患者给予膀胱综合训练方法,观察组患者在膀胱综合训练的基础上增加电针治疗。于治疗前、治疗4周后通过24h自主排尿次数、尿动力学检查、生活质量精简问卷(SQLP)评分评价2组患者膀胱功能情况。结果治疗4周后,观察组患者24h自主排尿次数、充盈期逼尿肌压力、残余尿量及SQLP评分均明显低于对照组水平,而最大膀胱测压容积及最大尿流率均明显高于对照组水平,差异均具有统计学意义(P<0.05)。结论电针联合膀胱综合训练可有效改善骶上脊髓损伤后神经源性膀胱(逼尿肌反射亢进型)患者膀胱功能,提高生活质量,该疗法值得临床推广。
Objective To observe the effects of electroacupuncture combined with bladder synthesis training on bladder function of patients with treating neurogenic bladder after supraorbital spinal cord injury.Methods A total of 50 cases of neurogenic bladder(detrusor hyperreflexia)after the spinal cord(cervical,thoracic,lumbar cord)injury were randomly divided into a control group(n=25)and an observation group(n=25).The control group was treated with bladder synthesis training,while the combination group was treated with electroacupuncture combined with bladder synthesis training.The bladder urination was tested using 24 h auto-urination times,urodynamic study and SQLP score before and after 4 weeks of treatment.Results After 4 weeks of treatment,the 24 h auto-urination times,detrusor pressure at filling time,residual urine volume and SQLP score in the observation group were significantly lower than those of the control group,while the average maximum bladder capacity and maximum urine flow rate of the observation group were significantly higher than those of the control group(P<0.05).Conclusion Electroacupuncture combined with bladder synthesis training could remarkably improve bladder function of patients with the neurogenic bladder(detrusor hyperreflexia)after supraorbital spinal cord injury.
