二维斑点追踪成像对非瓣膜性心房颤动患者左房功能的应用价值 被引量:4

Evaluation of left atrial function in patients with nonvalvular atrial fibrillation by two-dimensional speckle tracking imaging

摘要 目的:探讨二维斑点追踪成像(2D-STI)技术评价非瓣膜性房颤(NVAF)患者左房功能的应用价值。方法:选取2017年3月至2019年2月在广西医科大学第二附属医院收治的NVAF患者96例,分为左心耳无自显影组(A组,n=46)、左心耳自显影组(B组,n=30)、左心耳血栓组(C组,n=20),同期选取30例健康志愿者作为正常组。经胸超声心动图检查测量左房最大容积(LAVmax)、左房最小容积(LAVmin)并计算左房射血分数(LAEF);测量各组左房各壁收缩期应变率峰值(SRs)、舒张早期应变率峰值(SRe)和舒张晚期应变率峰值(SRa),并计算其平均值mSRs、mSRe、mSRa。经食管超声心动图检查测量左心耳血液最大排空速度(LAAEV)、最大充盈速度(LAAFV)及左心耳射血分数(LAAEF)。结果:A组、B组及C组LAVmax、LAVmin明显高于对照组,LAEF、mSRs、mSRe、mSRa明显低于对照组(P<0.05);A组、B组、C组LAVmax、LAVmin呈逐渐升高趋势,LAEF、mSRs、mSRe、mSRa、LAAEV、LAAFV、LAAEF呈逐渐降低趋势,差异均有统计学意义(均P<0.05)。结论:2D-STI技术可较好地评价NVAF的左房功能,并能间接反应左心耳的血流状态。 Objective: To explore the clinical value of two-dimensional speckle tracking imaging (2D-STI) on the evaluation of left atrial (LA) function in patients with nonvalvular atrial fibrillation(NVAF). Methods: A total of 96 patients with NVAF in our hospital from March 2017 to February 2019 were selected,and divided into group A ( n =46,LA appendage without spontaneous imaging),group B ( n =30,LA appendage with spontaneous imaging) and group C ( n =20,LA appendage thrombus).30 healthy volunteers were enrolled as a control group.The LA maximal volume(LAVmax),LA minimal volume(LAVmin),LA ejection fraction(LAEF),peak strain rate of each segment in the systolic,early and late diastolic period of LA (SRs,SRe and SRa) were measured by transthoracie echocardiography.The average strain rates(mSRs,mSRe and mSRa)were calculated.The LAA maximum emptying velocity(LAAEV),LAA maximum filling velocity (LAAFV),and LAA ejection fraction(LAAEF)were measured by transesophageal echocardiography. Results: Compared with the healthy controls,the LAVmax and LAVmin were significantly increased,while the LAEF,mSRs,mSRe and mSRa were significantly decreased in patients with NVAF.The LAVmax and LAVmin were higher in group B and C than those in group A,while the LAEF,mSRs,mSRe,mSRa,LAAEV,LAAFV and LAAEF were lower in group B and C,and the echocardiographic parameters were the highest or lowest in group C ( P <0.05). Conclusion: The 2D-STI technology could effectively evaluate the LA function and indirectly reflect the blood flow status of the LA appendage in patients with NVAF.
