摘要
目的分析扁桃体非霍奇金淋巴瘤(non-Hodgkin's lymphoma,NHL)的CT与MRI表现,以提高该病的影像诊断。方法回顾性分析经病理证实的25例扁桃体NHL的CT、MRI表现及病理结果。结果 25例中,单侧受累19例,双侧受累6例,共31个病灶,平均大小约21mm×27mm。27个为肿块型,21个边界清楚,6个边界不清;4个为浸润型,边界不清。18例行MRI检查,T1WI为等或稍低信号;T2WI为较高信号,15例信号均匀,3例内见小囊变、坏死区;5例行DWI检查,均为高信号。MRI增强扫描,15例均匀强化,3例不均匀强化,内见小斑片未强化区;1例轻度强化,13例中度强化,4例明显强化。9例行CT检查,平扫呈均匀等或稍低信号,增强扫描轻度强化。腭舌沟受累10例,咽旁间隙受累2例;16例出现颈部淋巴结肿大,单侧肿大12例,双侧肿大4例。结论扁桃体NHLCT及MRI表现有一定的特征,多表现为边界清楚、密度(信号)均匀的软组织肿块,轻-中度强化,咽旁受累少见;CT及MRI对该病的诊断及判断淋巴结受累具有重要价值。
Objective To investigate the CT and MRI findings of non-Hodgkin's lymphoma(NHL)in tonsil,to improve the imaging diagnosis of the disease.Methods The CT and MRI findings and pathological results of 25 NHL in tonsil were retrospectively analyzed.Results Among the 25 cases,19 cases were unilateral involvement and 6 cases were bilateral involvement.There were 31 lesions in total with an average size of 21mm×27mm.27 lesions showed mass type,with clear boundary in 21 lesions and unclear boundary in 6 lesions,4 lesions showed invasion type with unclear boundary.18 cases underwent MRI examination,T1WI showed iso-or hypointense,T2WI showed hyperintense.The signals were uniform in 15 cases,small patchy cystic or necrotic areas were observed in 3 cases.5 cases underwent DWI examination,all showed hyperintense.On contrast-enhanced MRI,homogenous enhancement were observed in 15 cases,heterogeneous enhancement with small patchy or necrotic areas were observed in 3 cases.Mild enhancement was observed in 1 case,moderate enhancement in 13 cases,and significant enhancement in 4 cases.9 cases underwent CT examination,iso-or low density was revealed on plain CT scan.Mild homogenous enhancement was observed in all cases.Palato-lingual sulcus were involved in 10 patients,parapharyngeal space were involved in 2 patients.Cervical lymph node enlargement occurred in 16 cases,including 12 unilateral cases and 4 bilateral cases.Conclusion The CT and MRI findings of NHL in tonsils showed certain characteristics.Most cases were showed soft tissue masses with clear boundaries and uniform density(signal).CT and MRI play important role in the diagnosis and discovery of lymph node involvement.
