术前影像改进在椎体成形术中的应用

摘要 目的:探讨术前影像改进应用于经皮椎体成形术术前计划及术中定位与传统方法行经皮椎体成形术的比较效果。方法:2018年7-12月选取的80例胸腰椎椎体骨折(T10~L5)疏松压缩性骨折患者,分为对照组及观察组,各40例。其中观察组40例患者在术前使用术前影像CT/MRI改进制定术前计划及术中定位,对照组40例行常规术中通过X线透视经椎弓根置入穿刺针,注入骨水泥,分析两组患者治疗效果差异。结果:观察组手术时间、椎体弥散程度、术后疼痛症状明显缓解率均优于对照组,差异均有统计学意义(P<0.05)。结论:通过术前影像CT/MRI改进制定术前计划及术中测量定位可以有效地减少手术时间,改善骨水泥在椎体的弥散,缓解疼痛。 Objective:To investigate the effect of preoperative imaging improvement on preoperative planning and intraoperative localization of percutaneous vertebroplasty compared with that of traditional percutaneous vertebroplasty.Method:From July to December 2018,80 patients with thoracolumbar vertebral fracture(T10-L5)were divided into the control group and the observation group,40 cases in each group.Forty patients in the observation group used preoperative CT/MRI to improve preoperative planning and intraoperative localization.In the control group,40 patients were injected with puncture needle and bone cement through pedicle of vertebral arch through X-ray fluoroscopy during routine operation.The difference of therapeutic effect between the two groups was analyzed.Result:The observation group was superior to the control group in terms of operation time,vertebral diffuse degree and pain relief after operation,the differences were statistically significant(P<0.05).Conclusion:Preoperative planning and intraoperative measurement and localization can effectively reduce the operation time,improve the diffusion of bone cement in vertebral body and relief pain by improving preoperative imaging CT/MRI.
