摘要
股骨头坏死(osteonecrosis of the femoral head, ONFH)是多种因素引起骨内血液供应不足或中断,出现骨细胞、骨髓基质细胞凋亡及骨髓组织成分部分坏死,继而引起骨代谢平衡紊乱的疾病。ONFH发病机制至今仍未完全阐明,其发病隐匿,进展快,早期治疗困难而致残率高。其临床治疗手段虽多,但疗效均难以令人满意,绝大多数患者最终都需要行髋关节置换,给患者家庭及社会造成巨大损失。近年来,研究发现IL-6在骨代谢平衡系统中,是促进骨吸收关键的炎性细胞因子,与许多疾病骨丢失,骨质破坏关系密切。因此,探讨IL-6与股骨头坏死的关系,临床上针对IL-6受体的靶向药物治疗股骨头坏死,有可能成为新的突破。
Osteonecrosis of the femoral head (ONFH) is a disease in which multi-factor-induced insufficient or interrupted blood supply to the bone leads to apoptosis of bone cells and bone marrow stromal cells and partial necrosis of the bone marrow tissue, which causes a disorder of bone metabolism. The pathogenesis of ONFH has not yet been fully elucidated, and ONFH has a high disability rate due to its occult development, rapid progress, and difficulty in early treatment. Although many clinical treatments are available for the disease, none of them can achieve a satisfactory effect. Most patients eventually need a hip replacement, which may cause great losses to patients' families and the society. In recent years, studies have found that IL-6 is a key inflammatory cytokine that can promote bone resorption in the bone metabolism system, and is closely related to bone loss and bone destruction in many diseases. Therefore, a breakthrough may be made by exploring the relationship between IL-6 and ONFH and adopting medications targeting IL-6 receptor for the treatment of ONFH in clinical practice.
