摘要
目的探讨DRGs收付费数据模拟运行对病案管理流程的影响,为优化病案管理提供数据和理论依据。方法对2018年11月30日-2018年12月31日实行DRGs收付费数据模拟运行期间与2017年同期出院病案归档时间、3个工作日归档率、病案编码完成时间、3个工作日编码完成率及不同付费方式下这些指标进行描述和统计分析。结果 DRGs试运行期间每份病案编码完成平均时间为3.30天,比运行前同期5.33天平均缩短约2天,差异有统计学意义(Z=51.096,P<0.001),北京医保患者病案3个工作日编码完成率为95.7%。此期间出院病案3个工作日归档率为96.30%,比之前同期的98.9%略有下降。结论 DRGs收付费数据模拟运行期间,不同付费方式的病案编码完成时间均显著缩短,但住院病案归档率略有降低,实行DRGs付费有利于病案管理流程的调整和优化。
Objectives To explore the influence of DRGs data simulation operation on medical record management process, providing data and theoretical basis for optimizing the management of medical records.Methods Compared the data of time from November 30, 2018 to December 31, 2018, when DRGs payment were simulated, with the data from the same period in 2017, describing these indicators, such as archiving time of inpatient cases, the 3 working days filing rate of inpatient cases, the time for the completion of the case code, the 3 working days code completion rate, and compared these indicators by different payment methods. Results The average time of completion of each case code during the DRGs trial operation was 3.30 days, which was approximately 2 days shorter than the average of 5.33 days before the same period of operation, and the difference was significant(Z = 51.096, P<0.001). The completion rate of the 3 working days code for the Beijing medical insurance case is 95.7 %. During this period, the filing rate of discharge cases for 3 working days was 96.30 percentage, which was a slight decrease from 98.9 percentage in the same period. Conclusions Compared with the non-implementation of DRGs collection and payment, the time for thcompletion of the clinical record coding of different payment methods had been significantly shortened, but the archiving rate of hospitalization cases had been slightly reduced, and the implementation of DRGs payment was conducive to the adjustment and optimization of the patient record management process.
