摘要
目的观察加巴喷丁联合高能红光治疗痛性糖尿病周围神经病变患者的疗效。方法将252例痛性糖尿病周围神经病变患者随机分为对照组、加巴喷丁组、高能红光组和联合组,每组63例。4组患者均给予常规疗法,在此基础上,加巴喷丁组给予口服加巴喷丁胶囊,高能红光组给予高能红光治疗仪治疗,联合组给予加巴喷丁联合高能红光治疗。比较4组患者疼痛视觉模拟量表(VAS)评分、生活质量评分、神经传导速度、治疗效果。结果治疗2周和4周时,联合组VAS评分均低于其他3组(均P<0.05)。治疗4周时,联合组患者正中神经中的运动神经传导速度快于其他3组,感觉神经传导速度快于对照组和加巴喷丁组(均P<0.05);联合组患者腓总神经的运动及感觉神经传导速度均快于其他3组(均P<0.05);联合组生活质量各项评分除社会维度评分外,均高于其他3组(均P<0.05);联合组和高能红光组患者疗效均优于对照组(均P<0.05)。4组患者不良反应发生率差异无统计学意义(P>0.05)。结论加巴喷丁联合高能红光治疗痛性糖尿病周围神经病变患者可以有效缓解疼痛,提高疗效及生活质量。
Objective To investigate the efficacy of gabapentin combined with high-energy red light therapy for patients with painful diabetic peripheral neuropathy. Methods A total of 252 patients with painful diabetic peripheral neuropathy were randomly divided into control group,gabapentin group,high-energy red light group and combination group, with 63 cases in each group.Patients in the four groups were treated with conventional therapy,based on which the gabapentin group received oral gabapentin capsules,the high-energy red light group was treated with the high-energy red light instrument,the combination group was given gabapentin capsules combined with high-energy red light therapy.Visual analogue scale(VAS) scores for pain,quality of life score,nerve conduction velocity,and therapeutic effect were compared among the four groups. Results After 2 and 4 weeks of treatment,the VAS scores in the combination group were lower than those in the other three groups(all P <0.05). After 4 weeks of treatment,the combination group had faster motor nerve conduction velocity of median nerve than the other three groups,and faster sensory nerve conduction velocity than the control group or the gabapentin group(all P <0.05);the combination group experienced faster motor and sensory nerve conduction velocities of common peroneal nerve than the other three groups(all P <0.05);the combination group exhibited higher scores for all issues concerning quality of life,except social dimension score,than the other three groups (all P <0.05);the efficacy in the combination group or high-energy red light group was superior to that in the control group (all P <0.05). There was no significant difference in incidence rate of adverse reactions among the four groups( P >0.05). Conclusion Gabapentin combined with high-energy red light therapy can relieve pain effectively, improve efficacy,and promote quality of life in the treatment of patients with painful diabetic peripheral neuropathy.
