宫颈HPV感染与阴道内细菌微环境/免疫微环境的关系 被引量:30

摘要 人乳头瘤病毒(HPV)是一种可导致女性宫颈癌的病毒,严重影响女性的身心健康和生殖结局,其感染和清除机制与宿主免疫功能的关系现尚未完全阐明。有相关研究显示HPV的感染与生殖道的内环境有关,内环境的紊乱与HPV感染有着一定的相关性。阴道内环境包括阴道及宫颈粘膜的免疫屏障和阴道内细菌群落,阴道内正常菌群由乳酸杆菌主导,可调节阴道内的免疫微环境,在一定程度上避免HPV的感染。在细菌性阴道病患者中,乳酸杆菌比例降低、其他细菌比例的增高可破坏阴道内微环境,从而增加HPV的易感性。此外,HPV相关的阴道粘膜免疫紊乱也与HPV感染有关,其中包含了相关免疫细胞的功能紊乱及数量减少,包括巨噬细胞、自然杀伤T细胞(NKTcells)、CD4^+和CD8^+T细胞;干扰素(IFN)等细胞因子的表达紊乱也与HPV感染相关。 HPV is a virus which can lead to cervical cancer,and it also affects women ’s physical and mental health. The relationship between the immunological function and the infection and elimination mechanism is not fully understood. Some studies have demonstrated that HPV is associated with the micro-environment of genital tract,and the disturbance of the micro-environment is related to HPV infection. The internal environment of vagina includes cervical mucosal immunity and bacterial flora. The normal flora in vagina is dominated by lactobacillus,which can modulate the micro-environment of local immunity and avoid normal cells from being infected by HPV. In the patients with bacterial vaginosis,the decrease of lactobacillus and increase of other flora may destroy the micro-environment,which leads to HPV infection. Moreover ,HPV-related mucosal immunity disturbance is another possible mechanism of HPV infection. The dysfunction and decrease number of immune cells including macrophages,NKT cell,CD4^+,CD8^+ T,as well as disorders of expression of cytokines such as interferon(IFN) are also associated to HPV infection.
