新生儿ABO溶血病微柱凝集法放散试验中放散液剂量优化研究 被引量:9

摘要 目的比较不同剂量放散液在ABO溶血病微柱凝集法放散试验中阳性检出率的差异,得到最佳放散液剂量。方法收集2018年1~9月,临床诊断符合ABO溶血病的新生儿血液标本,采用微柱凝集法,根据微柱凝胶卡反应腔容量,选取低、中、高三种剂量的放散液进行平行试验。结果微柱凝集法放散试验中不同剂量放散液(25μl组,50μl组,100μl组)的阳性检出率分别为82.09%,82.77%和99.55%;25μl与50μl两组间阳性检出率比较,χ^2=0.031 3,P>0.05,差异无统计学意义。100μl组与25μl和50μl两组间阳性检出率比较,差异均有统计学显著性意义(χ^2=78.519 6,74.948 6,均P<0.001),故100μl放散液组阳性检出率最佳。结论在ABO溶血病微柱凝集法放散试验中,推荐使用100μl剂量放散液,以提高放散试验阳性检出率。 Objective To compare the difference of positive detection rate of different doses of radiation solution in neonatal hemolytic disease serological test,and obtain the best dosage of radiation solution in erythrocyte antibody dispersal test.Methods From January 2018 to September 2018,neonates with jaundice or hyperbilirubinemia,incompatibility of maternal and infant blood groups,and clinical symptoms conforming to ABO-hemolysis were collected.Microcolumn agglutination method was used to test erythrocyte antibody diffusion in neonates with ABO-hemolytic disease.Three doses of low,medium and highwere selected and administered respectively.Parallel experiments were conducted and the results were analyzed statistically.Results Among 441 cases clinically diagnosed as ABO-hemolytic disease of newborn,the positive detection rates of different doses of radiation solution(25 μl,50μl and 100 μl groups) were 82.09%,82.77% and 99.55%.There was no significant difference between 25μl and 50 μl groups(χ^2=0.031 3,P>0.05).The positive detection rates of 100 μl group and other dosage groups were significantly different(χ^2=78.519 6,74.948 6,all P<0.001),and the positive detection rate of 100 μl radiation solution was the highest.Conclusion In the erythrocyte antibody diffusion test of ABO-hemolytic disease in neonates(microcolumn agglutination method),100 μl dose of radiation solution is recommended to improve the positive detection rate of hemolytic disease in neonates.
