摘要
目的:观察张仲景治疗水肿经方真武汤、越婢加术汤对阿霉素肾病模型大鼠肾组织B7-1表达的影响,探讨两种不同利水法疗效及中医"同病异治"分子机制。方法:雄性SD大鼠48只随机分为空白组、模型组、真武组及越婢组。采用尾静脉注射阿霉素建立肾病模型,分别灌胃相应药物。6周后检测各组大鼠24小时尿蛋白定量,肝功能:血清白蛋白(albumin,ALB)、谷丙转氨酶(alanine transaminase,ALT),肾功能:血肌酐(serum creatinine,Scr)、血尿素氮(urea nitrogen,BUN)及血脂:胆固醇(cholesterol,CHOL)、三酰甘油(triacylglycerol,TG)。HE染色观察大鼠肾脏病理;免疫组织化学法检测肾组织B7-1表达。结果:与模型组24小时尿蛋白定量[(0.40±0.82)g]比较,真武组[(0.18±0.88)g]和越婢组[(0.20±0.76)g]均降低(P<0.05);与模型组ALB[(12.30±1.74) g·L^-1]比较,真武组[(18.82 ±2.19)g·L^-1]和越婢组[(20.33±4.90)g·L^-1]均升高(P<0.05);与模型组Scr[(80.00±39.96)mmol·L-1]比较,真武组[(67.67±44.86)mmol·L^-1]和越婢组[(60.83±31.30)mmol·L^-1]均降低(P<0.05);与模型组血脂(CHOL、TG)比较,真武组和越婢组均下降(P<0.05)。肾脏病理:模型组可见部分肾小球肥大、少数系膜增生和肾小管病变,而真武组和越婢组改变均较轻微。与模型组B7-1水平[(79.37±4.24])比较,真武组[(56.82±5.52)]和越婢组[(52.38±3.61)]表达水平均较低(P<0.05)。结论:真武汤和越婢加术汤可能通过影响阿霉素肾病模型大鼠肾组织B7-1的表达,调节免疫而改善肝功能、肾功能,减轻肾脏病理损害。
Objective:To observe the effects of Zhenwu Decoction and Yuebi Jiazhu Decoction on the expression of B7-1 in renal tissue of model rats with adriamycin nephropathy treated by Zhang Zhongjing,and to explore the therapeutic effect of two different diuretic methods and the molecular mechanism of "treating the same disease differently" in Chinese medicine.Methods:48 male SD rats were randomly divided into blank group,model group,Zhenwu group and Yuebi group.Adriamycin was injected into tail vein to establish nephropathy model.The 24-hour urinary protein quantification,liver function:serum albumin(ALB),alanine transaminase(ALT),renal function:serum creatinine(Scr),urea nitrogen(BUN) and blood lipids:cholesterol(CHOL),triacyl glycerol(TG)were measured after 6 weeks of administration.HE staining was used to observe the pathology of rat kidney,and immunohistochemistry was used to detect the expression of B7-1 in renal tissue.Results:Compared with the 24-hour urine protein quantitation [(0.40±0.82) g] in the model group,the Zhenwu group [(0.18±0.88) g] and the Yuebi group [(0.20±0.76) g] were both decreased(P<0.05);compared with the ALB [(12.30±1.74) g·L^-1] in the model group,the Zhenwu group [(18.82±2.19) g·L^-1] and the Yuebi group [(20.33±4.90) g·L^-1] were both elevated(P<0.05);compared with the Scr[(80.00±39.96) mmol·L^-1] in the model group,the Zhenwu group[(67.67±44.86)mmol·L^-1] and the Yueqi group[(60.83±31.30) mmol·L^-1]were both decreased(P<0.05);compared with the blood lipids(CHOL,TG) in the model group,the Zhenwu group and the Yuebi group were both decreased(P<0.05).Kidney pathology:some glomerular hypertrophy,a few mesangial hyperplasia and tubular lesions were observed in the model group,while slight changes were observed in the Zhenwu group and the Yuemai group.Compared with the B7-1 level [(79.37±4.24]) in the model group,the expression levels of Zhenwu group [(56.82±5.52)] and Yuebi group [(52.38±3.61)] were lower(P<0.05).Conclusion:Zhenwu Decoction and Yuebi Jiashu Decoction may improve liver and kidney function and alleviate kidney pathological damage by affecting the expression of B7-1 in renal tissue of rats with adriamycin nephropathy and regulating immunity.
