摘要
目的观察小儿肺炎使用阿莫西林克拉维酸钾联合阿奇霉素的疗效及血清炎症因子水平。方法选取2015年5月至2018年5月浙江大学附属二院建德分院儿科收治的112例肺炎患儿,随机分为单一组和联合组,各56例。单一组:常规治疗+阿莫西林克拉维酸钾;联合组:常规治疗+阿莫西林克拉维酸钾+阿奇霉素。两组均以7d为一个疗程。观察两组患儿治疗前后白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及血清超敏C反应蛋白(hs-CRP)、氧化物歧化酶(SOD)、丙二醛(MDA)、一氧化氮(NO)及治疗后各症状消退时间,并记录不良反应。结果治疗后,联合组IL-6、TNF-α、hs-CRP水平明显低于单一组(t值分别为8.562、21.270、5.857,均P<0.05);治疗后,联合组SOD、MDA、NO水平较单一组明显改善(t值分别为6.408、6.763、8.245,均P<0.05);治疗后,联合组退热、肺啰音消失、扁桃体充血消退、咳嗽消失、用药的时间均低于单一组(t值分别为5.940、8.017、4.332、3.437、9.295,均P<0.05)。结论阿莫西林克拉维酸钾联合阿奇霉素治疗小儿肺炎能改善患者体内炎症及氧化应激水平,在缩短用药时间的同时改善患儿临床症状。
Objective To observe the efficacy and serum inflammatory factors levels of amoxicillin and clavulanate potassium combined with azithromycin in the treatment of infantile pneumonia.Methods Altogether 112 cases of children with pneumonia admitted to department of pediatrics of Jiande Branch of Second Affiliated Hospital of Zhejiang University from May 2015 to May 2018 were selected and randomly divided into single group and combined group,with 56 cases in each group.Single group was given conventional treatment with amoxicillin and clavulanate potassium,and combined group was given conventional treatment with amoxicillin and clavulanate potassium and azithromycin,and two groups were both treated with 7d as a course.Interleukin-6 (IL-6),tumor necrosis factor-α(TNF-α) and serum high-sensitivity C-reactive protein (hs-CRP),superoxide dismutase (SOD),malondialdehyde (MDA) and nitric oxide (NO) before and after treatment and time of symptoms remission after treatment were observed in two groups.And adverse reactions were recorded.Results After treatment,statistical levels of IL-6,TNF-α and hs-CRP in combined group were significantly lower than those in single group (t value was 8.562,21.270 and 5.857,respectively,all P<0.05).After treatment,statistical levels of SOD,MDA and NO in combined group were significantly improved compared with those in single group (t value was 6.408,6.763 and 8.245,respectively,all P<0.05).After treatment,time of defervescence,lung rales disappearance,tonsil congestion disappearance and cough disappearance and medication time in combined group were lower than those in single group (t value was 5.940,8.017,4.332,3.437 and 9.295,respectively,all P<0.05).Conclusion Amoxicillin and clavulanate potassium combined with azithromycin for infantile pneumonia can alleviate levels of inflammation and oxidative stress,and shorten medication time while improve clinical symptoms of children patients.
