摘要
目的 观察益气温阳活血方熏洗对糖尿病周围神经病变病人临床疗效及周围神经传导速度的影响。方法 选择我院2016年6月—2017年11月收治的120例糖尿病周围神经病变病人,采用随机数字表法,将病人分为对照组和观察组,各60例。两组均给予糖尿病常规治疗,对照组给予硫辛酸针治疗,观察组在对照组的基础上联合益气温阳活血方熏洗治疗。两组均以14d为1个疗程,治疗2个疗程后评定疗效。对两组病人治疗前后神经病变程度(MDNS)、中医证候和生活质量进行评分,检测周围神经传导速度,包括运动神经传导速度(MNCV)及感觉神经传导速度(SNCV),并评价临床疗效。结果 治疗后,两组病人MDNS评分和中医证候评分均较治疗前降低,其中观察组MDNS评分和中医证候评分均低于对照组,差异有统计学意义(P<0.05);两组周围神经传导速度明显升高,观察组MNCV和SNCV均高于对照组,差异有统计学意义(P<0.05);两组经过治疗后生活质量评分均升高,观察组生活质量评分高于对照组,差异有统计学意义(P<0.05);对照组总有效率为70.00%,低于观察组的91.67%,差异具有统计学意义(P<0.05)。结论 常规基础治疗联合中药益气温阳活血方熏洗治疗糖尿病周围神经病变,能够明显提高病人周围神经传导速度,改善病变程度,并提高生活质量。
Objective To study the effects of Yiqi Wenyang Huoxue recipe (YWHR) on clinical efficacy and peripheral nerve conduction velocity (NCV) in patients with diabetic peripheral neuropathy (DPN). Methods One hundred and twenty patients with DPN in our hospital from June2016 to November2017 were randomly divided into two groups:the control group (60 cases) treated with routine treatment plus lipoic acid,and the observation group (60 cases) treated with herbal fumigation treatment of YWHR plus lipoic acid and routine treatment for two courses of treatment,14 days for a cours of treatment.The Michigan Diabetic Neuropathy Score (MDNS),traditional Chinese medicine (TCM) syndrome and quality of life were observed.Peripheral NCV was detected,including motor NCV (MNCV) and sensory NCV (SNCV).The efficacy was evaluated. Results After treatment,the scores of MDNS and TCM syndrome were reduced in two groups,which were lower in the observation group than that in the control group ( P <0.05).The peripheral NCV was significantly increased in two groups.The MNCV and SNCV in the observation group were higher than those in the control group ( P <0.05).The quality of life scores were increased in the two groups after treatment,which were higher in the observation group than that in the control group ( P <0.05).The total clinical effective rate was significantly higher in observation group than that in the control group (91.67% vs.70.00%, P <0.05). Conclusion Herbal fumigation treatment of YWHR can significantly improve peripheral nerve conduction velocity,alleviate the degree of lesion and improve the quality of life in patients with DPN.
