摘要
目的分析乳腺癌的钼靶和磁共振征象,探讨其与分子分型的关系。材料与方法回顾性分析175例乳腺癌的钼靶与MRI征象,根据雌激素受体(estrogen receptor,ER)、孕激素受体(progesterone receptor,PR)、人类表皮生长因子受体-2(human epidermal growth factor receptor2,HER-2)及Ki-67的表达确定乳腺癌的分子亚型,分析影像征象与分子亚型的关系。结果LuminalB型和HER-2阳性型乳腺癌多有淋巴结转移,LuminalA型和三阴型乳腺癌淋巴结转移少(P<0.05)。HER-2过表达型乳腺癌多见可疑钙化,其他亚型乳腺癌钙化少见(P<0.05)。三阴型乳腺癌多表现为边缘强化肿块,其他亚型乳腺癌强化多不均匀(P<0.05)。结论乳腺癌的部分影像征象与分子亚型间有相关性,联合应用钼靶和MRI可初步判断乳腺癌的分子亚型,为临床诊断及个体化治疗提供参考。
Objective: To analyze the mammography and MRI features of breast cancer,and to explore the relationship between them and molecular subtypes.Materials and Methods: We retrospectively analyzed the mammography and MRI findings of 175 patients with breast cancer. According to the expression of estrogen receptor (ER),progesterone receptor (PR),human epidermal growth factor receptor-2 (HER-2) and Ki-67 determined the molecular subtypes of breast cancer and analyzed the relationship between imaging features and molecular subtypes. Results: Most of Luminal B and HER-2 positive breast cancer had lymph nodes metastasis, less Luminal A and triple negative breast cancer had node metastasis (P<0.05).Suspicious calcification was more common in HER-2 overexpression breast cancer than in other subtypes (P<0.05).Most of the triple negative breast cancer showed rim enhancement,while other subtypes showed heterogeneous enhancement (P<0.05).Conclusions: The phenotypes of mammography and MRI are diverse in different subtypes of breast cancer,and some features are related to the molecular subtypes.The combination of mammography and MRI can be used to distinguish the molecular subtypes of breast cancer and provide reference for clinical diagnosis and individualized treatment.
