摘要
目的:探讨枸橼酸铋钾联合奥美拉唑四联疗法治疗小儿消化性溃疡的临床效果及其对血清丙二醛(malondialdehyde, MDA)、脂质过氧化物(lipid peroxidation, LPO)和一氧化氮(nitric oxide, NO)的影响。方法:选取2016年11月-2017年11月收治的88例幽门螺杆菌(helicobacterpylori, Hp)阳性的消化性溃疡患儿,根据随机数字表法分为观察组和对照组,每组各44例。观察组采用枸橼酸铋钾联合奥美拉唑+克拉霉素+阿莫西林四联疗法,对照组采用奥美拉唑+克拉霉素+阿莫西林三联疗法,均治疗2周。比较两组临床疗效、Hp根除率、不良反应发生情况,检测治疗前后血清MDA、LPO和NO水平变化。结果:观察组、对照组治疗总有效率分别为90.91%(40/44)、75.00%(33/44),Hp根除率分别为93.18%(41/44)、70.45%(31/44),比较差异均有统计学意义( P <0.05)。与对照组比较,观察组治疗后血清MDA、LPO水平均明显降低,NO水平明显升高,差异有统计学意义( P <0.05);与本组治疗前比较,两组治疗后血清MDA、LPO水平下降,NO水平升高,差异有统计学意义( P <0.05)。观察组、对照组不良反应总发生率分别为27.27%(12/44)、18.18%(8/44),比较差异无统计学意义( P =0.309)。结论:枸橼酸铋钾联合奥美拉唑四联疗法治疗Hp阳性的小儿消化性溃疡的临床疗效显著,Hp根除率高,安全性好,可有效改善血清MDA、LPO、NO水平,减轻机体氧化应激反应。
Objective To investigate the clinical effect of bismuth potassium citrate combined with omeprazole quadruple therapy in the treatment of upper peptic ulcer in children and its effect on serum Malondialdehyde (MDA), Lipid Peroxidation (LPO), and Nitric oxide (NO). Methods A total of 88 peptic ulcer children with positive Helicobacterpylori (Hp) who were treated in our hospital from November 2016 to November 2017 were selected. According to the random number table method, they were divided into the observation group and the control group, with 44 cases in each group. The observation group was given bismuth potassium citrate combined with omeprazole + clarithromycin + amoxicillin quadruple therapy, and the control group was given omeprazole + clarithromycin + amoxicillin triple therapy. Both groups were treated for two weeks. The clinical efficacy, eradication rate of Hp and adverse reactions were compared between the two groups. The changes of serum MDA, LPO and NO levels before and after treatment were detected. Results The total effective rates of the observation group and the control group were 90.91%(40/44) and 75.00%(33/44), respectively, and the eradication rates of Hp were 93.18%(41/44) and 70.45%(31/44), respectively. The differences were statistically significant ( P <0.05). Compared with those in the control group, serum MDA and LPO levels were significantly decreased and NO levels were significantly increased in the observation group, suggesting significant differences ( P <0.05). As compared with those before treatment, the levels of serum MDA and LPO were decreased and the level of NO was increased after treatment. The difference was statistically significant ( P <0.05). The total incidence of adverse reactions in the observation group and the control group was 27.27%(12/44) and 18.18%(8/44), respectively, and there were significant differences ( P =0.309). Conclusion The clinical efficacy of bismuth potassium citrate combined with omeprazole quadruple therapy is significant in the treatment of upper peptic ulcer children with Hp-positive. The therapy has high eradication rates of Hp and good safety, which can also effectively improve serum MDA, LPO, NO levels, and reduce oxidative stress response of the body.
