周围型肺癌合并COPD患者低氧诱导因子水平变化及意义

摘要 目的分析周围型肺癌合并慢性阻塞性肺疾病(COPD)患者低氧诱导因子水平变化及意义。方法选取2017年6月至2018年10月本院收治的周围型肺癌患者152例,根据术前是否合并COPD分为COPD稳定期组76例、肺功能正常组76例。术中取患者肺组织标本,免疫组化法检测低氧诱导因子-1α(HIF-1α)阳性表达率;酶联免疫吸附法检测外周血血清及肺组织中HIF-1α浓度;术前检测患者肺功能和血气指标。结果 COPD稳定期组患者肺组织中HIF-1α阳性表达率[81.58%(62/76)]明显高于肺功能正常组[60.53%(46/76)],差异有统计学意义(P<0.05)。COPD稳定期组血清HIF-1α、肺组织匀浆HIF-1α水平均明显高于肺功能正常组,差异有统计学意义(均P<0.05)。肺功能正常组1秒用力呼气容积(FEV1)、FEV1/FVC水平明显高于COPD稳定期组,差异有统计学意义(P<0.05);肺功能正常组血氧分压(PaO2)水平明显高于COPD稳定期组,二氧化碳分压(PaCO2)水平明显低于COPD稳定期组,差异有统计学意义(均P<0.05)。血清和肺组织匀浆中HIF-1α水平与FEV1、FEV1/FVC、PaO2呈负相关(均P<0.05),与PaCO2呈正相关(P<0.05)。结论周围型肺癌合并COPD患者血清和肺组织中HIF-1α水平明显升高,与肺功能和血气指标明显相关,可能参与病情的发生、发展。
