摘要
目的:探讨社区2型糖尿病患者肾损伤与3项肥胖评价指标的相关性。方法:选取社区2型糖尿病患者465例,其中男性227例,平均年龄为(67.94±8.71)岁;女性238例,平均年龄为(66.87±7.23)岁。根据尿白蛋白/肌酐比值(UACR)分组,无蛋白尿组256例、微量蛋白尿组161例、大量蛋白尿组48例。分析糖尿病患者的肾功能与血糖、体质指数(BMI)、腰围(WC)、腰围身高比(WHt R)等指标的关系。结果:大量蛋白尿组和微量蛋白尿组患者的糖化血红蛋白(Hb A1c)、空腹血糖(FPG)、肌酐(Crea)、尿素氮(Urea)、尿微量白蛋白(UALB)均高于无蛋白尿组(P<0.05),估算肾小球滤过率(e GFR)低于无蛋白尿组(P<0.05)。微量蛋白尿和大量蛋白尿组患者的中心性肥胖患病率均高于无蛋白尿组(P<0.05)。相关性分析结果显示:3项肥胖评价指标均与eGFR呈负相关(P<0.01)。结论:社区2型糖尿病患者3项肥胖评价指标均与糖尿病肾损伤有关,提示糖尿病患者的体重控制有助于减小肾损伤的风险。
Objective: To explore the relationship between renal injury and three obesity evaluation indicators in type 2 diabetes patients in the community. Methods: Four hundred and sixty-five patients with type 2 diabetes in the community were selected, including 227 males with an average age of(67.94±8.71) years old and 238 females with an average age of(66.87±7.23) years old. According to the urinary albumin/creatinine ratio(UACR), they were divided into a non-proteinuria group with 256 cases, a microalbuminuria group with 161 cases and a high-proteinuria group with 48 cases. The relationship between renal function and blood glucose, body mass index(BMI), waist circumference(WC), and waist-to-height ratio(WHtR) were analyzed. Results: Glycated hemoglobin(HbA1c), fasting plasma glucose(FPG), creatinine(Crea), urea nitrogen(Urea), and urinary microalbumin(UALB) were higher in patients in the high proteinuria group and the microalbuminuria group than those in the non-proteinuria group(P<0.05), and the estimated glomerular filtration rate(eGFR) in the high proteinuria group and the microalbuminuria group was lower than that in the no-proteinuria group(P<0.05). The prevalence of central obesity in patients in the microalbuminuria group and the high proteinuria group was higher than that in the no-proteinuria group(P<0.05). The results of correlation analysis showed that the three obesity evaluation indicators were negatively correlated with eGFR(P<0.01). Conclusion: The three obesity indicators of type 2 diabetes mellitus in the community are all related to diabetic renal injury, and it is suggested that weight control in diabetic patients is helpful to reduce the risk of renal injury.
