血清IGFBP-3在新生儿甲状腺功能减退症替代治疗中的水平变化及其临床意义 被引量:6

摘要 目的探讨血清胰岛素样生长因子结合蛋白3(insulin-likegrowthfactorbindingprotein,IGFBP-3)在新生儿甲状腺功能减退症(甲减)替代治疗中的水平变化及其临床意义。方法回顾性选取2013年6月—2016年6月解放军联勤保障部928医院收治的甲减替代治疗新生患儿100例作为甲减组,同期选取健康新生儿100例作为对照组,检测所有新生儿血清IGFBP-3水平、甲状腺功能、身高、体质量。结果甲减组治疗前血清FT3、FT4、IGFBP-3水平、身高、体质量明显低于对照组,血清TSH水平明显高于对照组,差异有统计学意义(P<0.05);甲减组随访1年后血清FT3、FT4、IGFBP-3水平和身高、体质量明显高于治疗前,血清TSH水平明显低于治疗前,差异有统计学意义(P<0.05);随访1年后,甲减组身高、体质量和血清IGFBP-3水平明显低于对照组,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,血清IGFBP-3水平与血清FT4、FT3水平、身高、体质量呈正相关(P<0.05)。结论新生儿甲减可能造成机体IGFBP-3水平下降进而影响新生儿生长发育,经替代治疗后可一定程度上恢复IGFBP-3水平,但IGFBP-3水平可能仍偏低,提示应积极加强治疗。 Objective To study the changes and clinical significance of serum insulin-like growth factor binding protein 3(IGFBP-3)as an alternative therapy for neonatal hypothyroidism. Methods One hundred cases of newborns with hypothyroidism treated with the alternative therapy between June 2013 and June 2016 in our hospital were selected as the hypothyroidism group while another 100 healthy newborns served as the control group. Serum IGFBP-3 levels, thyroid function, height and weight were measured in all the neonates. Results Before treatment, serum levels of FT3, FT4 and IGFBP-3, height and weight in the hypothyroidism group were significantly lower than those of the control group, so were serum TSH levels, and,the difference was statistically significant(P<0.05). The serum levels of FT3, FT4 and IGFBP-3, height and weight were significantly higher in the hypothyroidism group during follow-up one year after treatment, while the serum TSH levels were significantly lower. The difference was statistically significant(P<0.05). After one-year follow-up, the height, weight and serum IGFBP-3 levels in the hypothyroidism group were significantly lower than those in the control group, and the difference was statistically significant(P<0.05). Pearson correlation analysis results showed that the levels of serum IGFBP-3 were positively correlated with the serum levels of FT3, FT4, height and body weight(P<0.05). Conclusion Hypothyroidism may cause the decrease of IGFBP-3 levels in the body, thereby affecting the growth and development of newborns.After the alternative therapy, the IGFBP-3 level can be restored to a certain extent.
