摘要
目的观察益气养阴经验方联合羟苯磺酸钙治疗轻、中度非增殖性糖尿病视网膜病变的临床疗效,及其对视觉电生理的影响。方法中医辨证为气阴两虚、络脉瘀阻证的轻、中度非增殖性糖尿病视网膜病变患者77例(148只眼),随机分为西药组、中药组和中西医联合组。西药组口服羟苯磺酸钙胶囊;中药组仅口服益气养阴经验方;中西医联合组同时服用两种药。治疗3个疗程,观察用药前后患者的视力、眼底情况、黄斑中心凹厚度、视觉电生理的潜伏期和振幅变化。结果3组治疗前后临床疗效比较均有统计学意义(P<0.05),中西医联合组治疗黄斑水肿疗效优于西药组与中药组,有统计学意义(均P<0.05)。3组患者的P50波、N95波及P100波的潜伏期较治疗前均缩短(均P<0.05),振幅较治疗前均上升(均P<0.05),且中西医联合组的潜伏期和振幅的改善情况均优于西药组与中药组(均P<0.05)。结论益气养阴经验方联合羟苯磺酸钙治疗轻、中度非增殖性糖尿病视网膜病变,在电生理方面有一定改善作用。
OBJECTIVE To observe the clinical efficacy of Yiqi Yangyin decoction combined with calcium dobesilate on mild and moderate non-proliferative diabetic retinopathy(NPDR)and its effect on visual electrophysiology,including pattern electroretinogram(PERG)and pattern visual evoked potential(PVEP).METHODS Seventy-seven patients with mild and moderate NPDR(148 eyes)in deficiency of qi and yin,and stasis obstructing collateral vessels syndromes were randomly divided into 3 groups.The western medicine group was treated with calcium dobesilate.The Chinese medicine group was treated with Yiqi Yangyin decoction.The integrated Chinese and western medicine group was treated with two methods above.All patients were treated for 3 courses,and the changes of clinical effects including visual acuity,fundus condition,macular fovea thickness,visual electrophysiology latency and amplitude were recorded and analyzed.RESULTS After 3 courses,three groups all received significant clinical effects with statistical evidence.The third group got better effect in terms of macular edema than other two groups(P<0.05).After treatment,the latency of P50 wave,N95 wave and P100 wave in three groups all decreased(P<0.05),the amplitude of them all elevated(P<0.05),and the third group had the best treatment efficacy in improving the latency and amplitude of P50 wave,N95 wave and P100 wave(P<0.05).The cu rative effect of the combined treatment group was the best(P<0.05).CONCLUSIONS The Yiqi Yangyin decoction combined with calcium dobesilate had a remarkable effect on mild to moderate non-proliferative diabetic retinopathy.
