摘要
目的观察自制新鲜食材匀浆膳对重症脑卒中合并2型糖尿病患者营养干预的效果。方法选取2018年1月至2018年7月重症脑卒中合并2型糖尿病患者66例,随机分为3组,分别给予自制新鲜食材匀浆膳(观察组)、低GI成品制剂(对照组Ⅰ)和家庭自制米糊(对照组Ⅱ),观察4周,记录每位患者营养支持前后的血糖指标和相关生化指标以及营养支持过程中胃肠道情况。结果经过4周营养支持后,观察组空腹血糖和餐后2 h血糖水平较营养干预前显著降低,同时改善了总蛋白、白蛋白、血红蛋白和肌酐等4项营养相关指标,差异有统计学意义(P<0.05)。与对照组Ⅱ相比,观察组也明显具有血糖控制和改善相关影响指标的效果(P<0.05)。虽然观察组的空腹血糖和餐后2 h血糖、总蛋白、白蛋白、血红蛋白和肌酐等指标水平与对照组Ⅰ没有显著性差异,但是不良胃肠道反应明显低于对照组Ⅰ,差异有统计学意义(P<0.05)。结论自制新鲜食材匀浆膳在控制重症脑卒中合并2型糖尿病患者血糖、不良胃肠反应及改善营养状况方面具有良好效果,且成本低、可个体化配制,值得临床推广和应用。
Objective To investigate the intervention effects of self-made fresh food homogenate meal on patients with severe cerebral stroke complicated by type 2 diabetes.Methods A total of 66 patients with severe cerebral stroke complicated by type 2 diabetes. Who were treated in our hospital from January 2018 to July 2018 were enrolled in the study,who were randomly divided into three groups,and the patients were given homemade fresh food homogenate meals(observation group), low GI product preparation(control groupⅠ) and homemade rice paste(control groupⅡ) for 4 weeks. The blood glucose indexes, related biochemical indicators and gastrointestinal tract reactions of patients before and after the intervention were observed and compared among the three groups.Results After 4-week nutritional support,the levels of fasting blood glucose and 2 h postprandial blood sugar in observation group were significantly decreased,as compared with those before intervention,at the same time, the nutrition related indexes including total protein, albumin, hemoglobin and creatinine in observation group were significantly improved(P<0.05).As compared with those in control groupⅡ,the blood glucose indexes in observation group were also significantly improved(P<0.05). Although there were no significant differences in the levels of fasting blood glucose,2 h postprandial blood sugar,total protein, albumin, hemoglobin between observation group and control groupⅠ,however,the incidence rate of gastrointestinal adverse reactionsin observation group was significantly lower than that in control groupⅠ(P<0.05).Conclusion The homemade fresh food homogenate meal has obvious effects in controlling blood sugar, improving nutrition status and controlling gastrointestinal adverse reactions in patients with severe cerebral stroke complicated by type 2 diabetes,with lower cost and individualized preparation, therefor, which is worth using widely in clinical practice.
