钆塞酸二钠增强磁共振成像在慢性肝炎背景下小肝癌诊断中的应用价值 被引量:5

摘要 目的探讨钆塞酸二钠(Gd-EOB-DTPA)增强MRI在慢性肝炎背景下小肝癌诊断中的应用价值。方法选取2017年4月至2018年8月收治的慢性肝炎合并肝占位性病变患者40例作为研究对象,统计其Gd-EOB-DTPA增强MRI扫描的诊断数据,以术后病理检查联合DSA造影检查结果为金标准,分析其平扫、三期增强图像及肝细胞期图像,并统计其诊断准确率、灵敏度和特异度。结果术后病理检查联合DSA造影检查显示,40例患者中有29例小肝癌结节,其中典型结节20例,不典型结节9例。Gd-EOBDTPA增强MRI检查中,典型结节直径(1.42±0.48)cm,不典型结节直径(0.87±0.28)cm;不典型结节病理分期为Ⅰ~Ⅱ期,典型结节病理分期为Ⅱ~Ⅲ期。Gd-EOB-DTPA增强MRI的灵敏度、特异度和准确率分别为93.10%、90.91%和92.50%。结论在慢性肝炎合并小肝癌患者的早期诊断中,Gd-EOB-DTPA增强MRI检查能够有效观察到结节的大小和血流信号,有利于提高临床诊断准确率。
