摘要
目的探讨盆底超声在产后早期压力性尿失禁中的应用价值及可行性分析。方法选择本院2017年8月至2018年7月间收治的产后尿失禁患者40例作为研究组,另外选取同期40例正常产后妇女作为对照组,两组患者均接受盆底超声检查,比较两组患者尿道膀胱后角(Ar、As)、膀胱颈旋转角(θ)、膀胱颈至耻骨联合中点的距离(Br、Bs)、膀胱颈移动度(Mu)以及尿道内口形成情况。结果研究组患者Ar、As、θ等显著大于对照组,Br、Bs及Mu等显著长于对照组(P<0.05);研究组尿道内口形成率为85.00%(34/40)明显高于对照组尿道内口形成率12.50%(5/40)(P<0.05)。结论产妇产后早期盆底超声检查的超声参数可有效反应产妇尿道、膀胱支持结构与功能变化情况,在产后早期压力性尿失禁的临床诊断及治疗中具有重要的应用价值。
Objective To investigate the application value and feasibility of pelvic floor ultrasound in early postpartum stress urinary incontinence. Methods 40 cases of postpartum urinary incontinence admitted to our hospital from August 2017 to July 2018 were selected as the study group, and 40 normal postpartum women were selected as the control group at the same time. The two groups of patients underwent pelvic floor ultrasonography. The posterior horn of urethra and bladder (Ar, As), rotation angle of bladder neck (theta), bladder neck to pubic symphysis were compared between the two groups. The distance from the midpoint (Br, Bs), the bladder neck mobility (Mu) and the formation of the urethral opening. Results Ar, As, and theta in the study group were significantly higher than those in the control group, Br, Bs and Mu were significantly longer than those in the control group (P<0.05);the formation rate of urethral orifice in the study group was 85.00%(34/40) significantly higher than that in the control group 12.50%(5/40), the difference was statistically significant (P<0.05). Conclusion Ultrasound parameters of early postpartum pelvic floor ultrasonography can effectively reflect the changes of urethral and bladder support structure and function. It has important application value in the clinical diagnosis and treatment of early postpartum stress urinary incontinence.
