国产氟康唑和进口氟康唑对艾滋病合并念珠菌感染的抗菌疗效及免疫功能分析 被引量:2

摘要 目的 探讨国产氟康唑和进口氟康唑对艾滋病合并念珠菌感染的抗菌疗效及免疫功能的影响。方法 选取2013年1月—2018年4月我院收治确诊的488例艾滋病合并念珠菌感染患者作为研究对象,通过随机数字表法分为对照组与观察组,病原菌培养类型均为念珠菌。在常规治疗基础上,对照组给予国产氟康唑治疗,观察组给予进口氟康唑治疗,均连续治疗15d。比较2组抗菌疗效、不同时间点CD4+T淋巴细胞计数的变化及治疗期间不良反应的发生情况。结果 对照组与观察组各入组244例患者,其中对照组有4例因自身原因退出研究。治疗后,观察组抗菌临床疗效总有效率为72.95%(178/244),明显高于对照组的64.17%(154/240)(P<0.05);2组CD4+T淋巴细胞计数较治疗前均有显著升高(P<0.05),且观察组治疗后10d、20d时CD4+T淋巴细胞计数均明显高于对照组,差异具有统计学意义(P均<0.05);治疗期间,观察组不良反应总发生率为15.98%(39/244),明显低于对照组的24.17%(58/240)(P<0.05)。结论 进口氟康唑对艾滋病合并念珠菌感染的抗菌疗效显著,有助于改善患者免疫功能,且不良反应少,安全性高。 Objective To study the antibacterial efficacy of domestic and imported fluconazole in treatment of AIDS complicated with Candida infection and its effect on immune function. Methods Four hundred and eighty-eight patients of AIDS complicated with Candida infection who were diagnosed and treated in our hospital from January 2013 to April 2018 were selected as research objects. The included patients were divided into control group and observation group according to random digital table method. The pathogenic bacteria culture type was the Candida. On the basis of routine treatment, the control group was treated with domestic fluconazole, while the observation group was treated with imported fluconazole for 15 days. The antibacterial efficacy, the changes of CD4+ T lymphocyte count at different time points and the incidence of adverse reactions during treatment in the 2 groups were compared. Results There were 244 patients enrolled in each of control group and observation group, of which 4 patients in the control group withdrew from the study due to personal reason. After treatment, the overall response rate of antibacterial efficacy in the observation group was 72.95%(178/244), which was significantly higher than that of the control group 64.17%(154/240)(P < 0.05). The CD4+ T lymphocyte count of the 2 groups increased significantly compared with that before treatment (P < 0.05), and the CD4+ T lymphocyte count in the observation group was significantly higher than that of the control group at 10 days and 20 days after treatment, the difference was statistically significant (P < 0.05). During treatment, the total incidence of adverse reactions in the observation group was 15.98%(39/244), which was significantly lower than that in the control group 24.17%(58/240)(P < 0.05). Conclusions Imported fluconazole has a significant antimicrobial efficacy in the treatment of AIDS complicated with Candida infection, and is conducive to improving immune function, with fewer adverse reactions and high safety.
