高频率叩背联合手法震动按压对胸腔镜下肺叶切除术后排痰效果及并发症的影响 被引量:11

摘要 目的探讨高频率叩背联合手法震动按压对胸腔镜下肺叶切除术后排痰效果及并发症的影响。方法选自我院于2016年5月至2018年5月期间行胸腔镜下肺叶切除术后患者94例作为研究对象,按照随机表法分为观察组47例与对照组47例。对照组采用传统背部叩击手法排痰,观察组采用高频率叩背联合手法震动按压。比较两组24h排痰量、胸管拔除时间、住院时间、术后离床活动时间和术后活动开始时间,干预前后呼吸频率变化,及并发症发生情况。结果观察组24h排痰量多于对照组(P<0.05)。观察组胸管拔除时间快于对照组,住院时间短于对照组(P<0.05)。观察组术后离床活动时间和术后活动开始时间快于对照组(P<0.05)。两组干预后呼吸频率降低(观察组:t=17.206,对照组:t=10.373,P<0.05);观察组干预后呼吸频率低于对照组(t=12.380,P<0.05)。观察组并发症(4.25%)低于对照组(21.28%)(P<0.05)。结论高频率叩背联合手法震动按压,对胸腔镜下肺叶切除术后排痰效果显著,且并发症少,值得临床借鉴。 Objective To investigate the effect of high frequency percussion combined with manipulative vibration compression on sputum excretion and complications after thoracoscopic lobectomy.Methods 94 patients who underwent thoracoscopic lobectomy in our hospital from May 2016 to May 2018 were selected as the study subjects,including 47 patients in the observation group and 47 patients in the control group.The control group used the traditional back tapping expectoration,and the observation group used high-frequency tapping combined with manipulative vibration compression.The amount of sputum excreted at 24 hours,the time of thoracic duct extraction,hospital stay,the time of ambulation after operation and the beginning time of ambulation after operation,the change of respiratory rate before and after intervention,and the occurrence of complications were compared between the two groups.Results The sputum excretion rate of 24h in the observation group was higher than that in the control group(P<0.05).The extubation time of the observation group was faster than that of the control group,and the duration of hospital stay in the observation group was shorter than that in the control group(P<0.05).The ambulation time and postoperative activity time in the observation group were faster than those in the control group(P<0.05).Respiratory frequency of the two groups decreased after intervention(the observation group:t=17.206;the control group:t=10.373,P<0.05).The respiratory rate of the observation group was lower than that of the control group after intervention(t=12.380,P<0.05).The complication rate in the observation group(4.25%)was lower than that in the control group(21.28%)(P<0.05).Conclusion High frequency percussion combined with manipulative vibration compression is effective in expectoration after thoracoscopic lobectomy,and it has fewer complications,which is worthy of clinical reference.
