摘要
目的探讨臀肌筋膜挛缩症(gluteal muscle contracture, GMC)影像学特点。方法回顾分析388例经临床确诊GMC患者的临床及影像学资料。388例术前行骨盆X线及CT检查,其中2例行双侧髋关节MRI检查。378例接受手术治疗,其中129例术后行CT检查,3例行MRI检查。结果患者主要临床表现为髋关节弹响、活动受限及并脚下蹲困难。X线:373例(96.1%)见骨盆致密线;CT:388例(100%)见臀肌“挛缩带”、臀肌萎缩;MRI:2例(100%)见双低信号臀肌“挛缩带”。70例(18.0%)见骨盆倾斜。臀大肌挛缩产生骨盆致密线,臀中肌挛缩导致髂骨外翻。术后CT及MRI见臀肌挛缩带中断。结论GMC在CT或MRI检查可见特征性臀肌“挛缩带”,指导临床治疗并进行术后影像学评估。
Objective To investigate the image findings of gluteal muscle contracture (GMC). Methods Imaging and clinical data of 388 patients with GMC confirmed by clinical were reviewed retrospectively. All of 388 patients had pelvis X-ray examinations and CT scans before arthroscopic surgery, 2 of 388 patients had MRI scans of hips. 378 of 388 patients had surgical treatment, 129 patients of them had CT scans afteRsurgery and 3 of them were subjected to MRI scans. Results Snapping hip, limitation of hip movement and difficulty of squat with parallel foot were the main clinical manifestation of these patients. Pelvis plane films showed: Iliac hyperdense line was found in 373 patients (96.1%). Pelvis CT scans showed: fibrous bands and atrophy gluteal muscles were found in 388 patients (100%). MRI scans of hips shwoed: the hypointensity fibrous bands were shown in 2 patients (100%). 70 patients (18.0%) accompanied with pelvic obliquity. Iliac hyperdense line, snapping hip and limitation of the hip activity were related to glutes maximus contracture and ilium ectropion was due to contracture of gluteus medius. Postoperative CT and MRI scans showed fibrous bands were cut off. Conclusion The typical fibrous bands of GMC can be shown by CT and MRI, which could provide help foRclinical surgical treatment and evaluation of the effect of operation.
