咳嗽变异性哮喘患儿血中嗜酸性粒细胞和免疫球蛋白的关系研究 被引量:3

摘要 目的探讨给予咳嗽变异性哮喘(CVA)的患者血中嗜酸性粒细胞(EOS)和免疫球蛋白相关联系。方法随机抽取选取2017年12月-2018年12月该院收治的55例咳嗽变异性哮喘患儿作为临床观察对象。根据患儿血清中特异性免疫球蛋白E(SIgE)检测结果的不同,分为A组(血清中特异性免疫球蛋白E检测结果为强阳性,30例)与B组(血清中特异性免疫球蛋白E检测结果为弱阳性,25例),再选取20名健康儿童为对照组。对比观察3组患者血清总免疫球蛋白E(TIgE)与血中嗜酸性粒细胞(EOS)的关系及血清中特异性免疫球蛋白E(SIgE)抗体的阳性率结果分析。结果A组免疫球蛋白(550.32±42.24)mg/dL明显高于B组免疫球蛋白(44.24±5.86)mg/dL,差异有统计学意义(t=64.8781,P<0.05);A组嗜酸性粒细胞(6.38±1.23)%结果明显高于B组嗜酸性粒细胞(1.68±0.06)%,差异有统计学意义(t=20.8994,P<0.05)。A组中特异性免疫球蛋白E抗体的阳性率排列顺序为:户尘螨95%、鸡蛋白53%、花粉42%、羊肉20%、虾蟹15%、真菌8%。B组中特异性免疫球蛋白E抗体的阳性率排列顺序为:花粉57%、鸡蛋白45%、户尘螨43%、羊肉36%、真菌12%、虾蟹8%。A组中特异性免疫球蛋白E抗体的阳性重叠率结果为:户尘螨81%、鸡蛋白53%、花粉42%、羊肉27%、虾蟹16%、真菌6%。B组中特异性免疫球蛋白E抗体的阳性重叠率结果为:花粉45%、鸡蛋白45%、户尘螨48%、羊肉26%、真菌10%、虾蟹12%。结论咳嗽变异性哮喘(CVA)的患者血中嗜酸性粒细胞(EOS)和免疫球蛋白含量显著升高与血清中特异性免疫球蛋白E(SIgE)结果为强阳性者相关联。SIgE检测与EOS和免疫球蛋白检测比较作用较为全面,疾病诊断率更高,起到较好的临床预防作用。 Objective To investigate the relationship between eosinophils (EOS) and immunoglobulin in patients with cough variant asthma (CVA). Methods A total of 55 children with cough variant asthma admitted to our hospital from December 2017 to December 2018 were selected as clinical observation subjects. According to the results of specific immunoglobulin E (SIgE) test in children's serum, they were divided into group A (strong immunoglobulin E test results in serum, 30 cases) and group B (weakly positive strong immunoglobulin E test results in serum, 25 cases). And 20 healthy children were selected as the control group. The relationship between serum total immunoglobulin E (TIgE) and eosinophils (EOS) in blood and the positive rate of specific immunoglobulin E (SIgE) antibody in serum were analyzed. Results Immunoglobulin in group A (550.32±42.24) mg/dL was significantly higher than that in group B (44.24±5.86) mg/dL, and the difference was statistically significant (t=64.878 1,P<0.05). Eosinophils of group A (6.38±1.23)% was significantly higher than that of group B(1.68±0.06)%, and the difference was statistically significant (t=20.899 4,P<0.05). In group A, the positive rate of specific immunoglobulin E antibody was ranked as follows: household dust mites 95%, chicken protein 53%, pollen 42%, mutton 20%, shrimp and crab 15%, fungi 8%. The positive rate of specific immunoglobulin E antibody in group B was as follows: pollen 57%, chicken protein 45%, house dust mites 43%, mutton 36%, fungi 12%, shrimp and crab 8%. The positive overlap rate of specific immunoglobulin E antibodies in group A was 81% for household dust, 53% for chicken protein, 42% for pollen, 27% for mutton, 16% for shrimp and crab, and 6% for fungi. The positive overlap rate of specific immunoglobulin E antibodies in group B was: pollen 45%, chicken protein 45%, house dust mites 48%, mutton 26%, fungi 10%, shrimp and crab 12%. Conclusion For the patients with cough variant asthma (CVA), significantly elevated levels of eosinophils (EOS) and immunoglobulin are associated with strong positive immunoglobulin E (SIgE) results in serum. The comparison of SIgE detection with EOS and immunoglobulin detection is more comprehensive, and the disease diagnosis rate is higher, which plays a better clinical preventive role.
