摘要
目的观察老年原发性高血压患者血浆β2微球蛋白(β2-Microglobulin,β2-MG)、踝肱指数(ankle brachial index,ABI)变化及其与血压变异性(blood pressure variability,BPV)的相关性。方法选取2017年1月—2018年7月保定市第二医院老年病科住院的老年原发性高血压患者423例作为观察组,另选取同期于我院体检中心行健康体检的身体健康老年人120例作为对照组,比较两组β2-MG、ABI水平变化。另根据β2-MG、ABI水平的不同将观察组又分为高低β2-MG、ABI亚组,观察高低β2-MG、ABI亚组间BPV的差异;并进一步分析患者β2-MG、ABI水平与BPV的相关性。结果观察组β2-MG水平高于对照组,ABI水平低于对照组,差异均有统计学意义(P<0.05)。高β2-MG亚组患者24h收缩压变异性(24h systolic blood pressure variability,24hSBPV)、24h舒张压变异性(24h diastolic blood pressure variability,24hDBPV)及24h平均动脉压(24h mean artery blood pressure,24hMABP)均高于低β2-MG亚组患者,差异均有统计学意义(P<0.05或P<0.01);低ABI亚组患者24hSBPV、24hDBPV及24hMABP水平均高于高ABI亚组患者,差异均有统计学意义(P<0.05或P<0.01)。Cox回归模型分析结果显示,β2-MG与24hSBPV、24hDBPV均呈正相关,ABI与24hSBPV、24hDBPV均呈负相关。结论老年原发性高血压患者血浆β2-MG水平升高,ABI水平降低,β2-MG、ABI与老年原发性高血压BPV具有相关性,可一定程度影响BPV。
Objective To observe the changes of plasma β 2-Microglobulin (β 2-MG), ankle brachial index (ABI) and their correlation with blood pressure variability (BPV) in elderly patients with essential hypertension. Methods A total of 423 elderly patients with essential hypertension hospitalized in Geriatrics Department of Baoding Second Hospital from January 2017 to July 2018 were selected as the observation group, and 120 healthy elderly people who underwent physical examination during the same period were selected as the control group. The changes of β 2-MG and ABI levels were compared between the two groups. In addition, according to the different levels of β 2-MG and ABI, the observation group was divided into high and lowβ 2-MG and ABI subgroups. The differences of BPV between high and low β 2-MG and ABI subgroups were observed, and the correlation between β 2-MG, ABI levels and BPV was further analyzed. Results The level of β 2-MG in the observation group was higher than that in the control group, and the level of ABI was lower than that in the control group, suggesting significant differences ( P <0.05). The 24 h systolic blood pressure variability (24 h SBPV), 24 h diastolic blood pressure variability (24 h DBPV) and 24 h mean arterial pressure (24 h MABP) levels in the highβ 2-MG subgroup were higher than those in the low β 2-MG subgroup, and there was statistical significance ( P <0.05 or P <0.01).24 h SBPV, 24 h DBPV and 24 h MABP levels in low ABI subgroup were higher than those in high ABI subgroup, and significant difference was found ( P <0.05 or P <0.01). Cox regression model analysis showed that β 2-MG was positively correlated with 24 h SBPV and 24 h DBPV, while ABI was negatively correlated with 24 h SBPV and 24 h DBPV. Conclusion The level of plasma β 2-MG is elevated and ABI is decreased in elderly patients with essential hypertension. There is a correlation between β 2-MG, ABI and BPV in elderly patients with essential hypertension, which can affect BPV to a certain extent.
