摘要
目的比较观察微创拔牙与传统拔牙方法在下颌阻生智齿拔除中的效果与安全性,为今后的手术治疗方法选择提供参考。方法选取我院及惠州市第一人民医院口腔科2016年1月~2019年4月收治的5000例单侧下颌阻生智齿拔除患者作为观察样本,采用随机双盲法将其分为对照组与观察组,各2500例。对照组采用传统拔牙(凿骨劈冠法)治疗,观察组采用高速涡轮机、长裂钻头、微创拔牙刀相结合的微创拔牙治疗。比较两组的手术时间,并采用视觉模拟评分法对两组患者的术后疼痛程度进行评估,记录两组围术期并发症的发生情况。结果观察组的手术时间为(17.75±3.53)min,明显短于对照组的(33.16±6.24)min,差异有统计学意义(P<0.05);观察组的术后疼痛程度明显优于对照组,差异有统计学意义(P<0.05);观察组围术期的并发症总发生率为19.16%,明显低于对照组的50.32%,差异有统计学意义(P<0.05)。结论微创拔牙技术在下颌阻生智齿拔除中的手术时间更短,术后疼痛程度更轻,而且围术期的并发症发生率更低,安全性高,可以作为首选方法在临床中推广应用。
Objective To compare and observe the effect and safety of minimally invasive extraction and traditional extraction in the extraction of impacted mandibular wisdom teeth,so as to provide reference for the selection of surgical treatment methods in the future.Methods A total of 5000 patients with unilateral impacted mandibular wisdom teeth in the Department of Stomatology of our hospital and the First People's Hospital of Huizhou City from January 2016 to April 2019 were selected as observation samples.They were randomly divided into the control group and the observation group,with 2500 cases in each group.The control group was treated with traditional tooth extraction (chisel and crown splitting),and the observation group was treated with high-speed turbines and long splitting,minimally invasive tooth extraction combined with drill and minimally invasive tooth extraction knife.The operation time of the two groups was compred,and visual analogue scale score was used to evaluate the degree of pain after operation in the two groups.The complications of the two groups were recorded the occurrence of perioperative complications.Results The operation time of the observation group was (17.75±3.53) min,which was significantly shorter than (33.16±6.24) min of the control group,with statistical significance (P<0.05).The pain degree of the observation group was significantly better than that of the control group,with statistical significance (P<0.05).The total incidence of perioperative complications in the observation group was 19.16%,which was significantly lower than that in the control group (50.32%),with statistical significance (P<0.05).Conclusion Minimally invasive extraction of impacted mandibular wisdom teeth has shorter operation time,with less pain,lower incidence of perioperative complications and high safety.It can be used as the preferred method in clinical promotion and application.
