摘要
目的:评估RNA实时荧光恒温扩增技术(RNA-SAT)在儿童肺炎支原体(MP)感染中的应用价值及与MP-IgM抗体检测的差异性。方法:收集近2年安徽医科大学第一附属医院儿童呼吸病区送检的临床标本经SAT检测为MP-RNA阳性的菌株。对阳性株的标本来源、患儿年龄、性别、送检MP-IgM时间、结果、相关实验室指标及临床诊断进行统计分析。结果:181株经SAT检验为阳性的MP-RNA标本来源于155例患儿的咽拭子、痰液及肺泡灌洗液。MP感染患儿中位年龄为4岁,年龄均数及标准差为(4.78±2.957)岁;151例送检IgM抗体的患儿中,<5岁与≥5岁组间和≤7天、8~13天及≥14天组间IgM的阳性率比较差异有统计学意义(P<0.05);肺炎支原体肺炎(MPP)组与非MPP组间白细胞计数、中性粒细胞及淋巴细胞比例差异有统计学意义(P<0.05);难治性肺炎支原体肺炎(RMPP)组与普通型肺炎支原体肺炎(GMPP)组间IgM抗体阳性率比较差异无统计学意义(P>0.05)。结论:MP-IgM抗体检测阳性率与患儿年龄及采集标本的时间密切相关。RNA-SAT在儿童MP感染中的诊断价值较高,多次送检临床标本及联合多种实验室诊断有利于提高MP的检出率,指导临床诊断及治疗。
Objective: To evaluate the application value of RNA-SAT in mycoplasma pneumoniae (MP) infection in children and the difference between it and MP-IgM antibody detection. Methods: Clinical specimens from the children's respiratory ward of The First Affiliated Hospital of Anhui Medical University in recent two years were collected. The source of samples, age, gender, time and results of MP-IgM examination, relevant laboratory indexes and clinical diagnosis of the positive strains were statistically analyzed. Results: 181 MP-RNA positive samples were obtained from pharyngeal swabs, sputum and bronchoalveolar lavage fluid of 155 children. The median age of MP infected children was 4 years old, and the mean age and standard deviation were 4.78±2.957 years. Among 151 children with IgM antibody, there were statistically significant differences in positive rates of IgM between groups younger than 5 years old and groups older than or equal to 5 years old. The same results were detected between groups younger than or equal to 7 days, 8~13 days and groups older than or equal to 14 days. There were statistically significant differences in white blood cell count, neutrophil and lymphocyte proportion between MPP and non-MPP groups. Conclusion: The positive rate of MP-IgM antibody detection is closely related to the age of children and the time of specimen collection. The diagnostic value of RNA-SAT in MP infection in children is relatively high.Multiple submissions of clinical samples combined with a variety of laboratory diagnoses are conducive to improving the detection rate of MP and guiding clinical diagnosis and treatment.
