摘要
目的探讨扩散峰度成像(DKI)定量参数在诊断宫颈鳞癌分级中的价值,及其与鳞状细胞癌抗原(SCC-Ag)的相关性。方法回顾性分析36例宫颈癌鳞癌患者的DKI影像资料,以病理结果为金标准,分为低分化宫颈鳞癌组(n=22)和高中分化宫颈鳞癌组(n=14),分别测量各组肿瘤实质区平均扩散峰度(MK)值、平均扩散系数(MD)值及各项异性分数(FA)值。应用化学发光法(CLIA)测定血清SCC-Ag浓度。对比低分化与高中分化宫颈鳞癌组间DKI各参数值及SCC-Ag表达水平的差异,评价其诊断阈值及效能,并分析各参数与SCC-Ag的相关性。结果低分化宫颈鳞癌组MK值、FA值、SCC-Ag水平高于高中分化宫颈鳞癌组(P<0.05),MD值低于高中分化宫颈鳞癌组(P<0.05),诊断阈值分别为0.89、0.13、3.81ng/ml和1.00×10^-3mm^2/s,ROC曲线下面积分别为0.815、0.726、0.795、0.769;MK值与SCC-Ag呈正相关(r=0.525,P<0.05),MD值与SCC-Ag呈负相关(r=-0.658,P<0.01),FA值与SCC-Ag无显著相关性(P>0.05)。结论DKI有助于鉴别低分化与高中分化宫颈鳞癌,其参数MK、MD值与SCC-Ag具有相关性。
Objective To investigate the value of quantitative parameters of diffusion kurtosis imaging (DKI) in differentiating pathological grades of squamous cell carcinoma (SCC) of cervix and the correlations between serum SCC antigen (Ag).Methods The DKI data of 36 patients with cervical SCC were analyzed retrospectively.According to the pathological results,they were divided into poorly differentiated (22) and well/moderately differentiated (14) groups.The mean kurtosis (MK),mean diffusion (MD) and fractional anisotropy (FA) values of the SCC were determined.Chemiluminescence immunoassay (CLIA) was used to measure the serum concentration of SCC-Ag.The DKI parameters and SCC-Ag expression levels between the two groups were compared.Results The MK,FA,and serum SCC-Ag values were significantly (P<0.05) higher whereas the MD value was significantly (P<0.05) lower in the poorly differentiated than that of well/moderately differentiated tumor groups.The diagnostic threshold was 0.89 (MK),0.13 (FA),3.81 ng/ml (SCC-Ag),and 1.00×10^-3 mm^2/s with area under the ROC curves of 0.815,0.726,0.795,and 0.769,respectively.There was a positive correlation between MK and SCC-Ag (r=0.525,P=0.01),negative correlation between MD and SCC-Ag (r=-0.658,P<0.01) and no significant correlation between FA and SCC-Ag (P>0.05).Conclusion DKI is useful in distinguishing the poorly differentiated from the well/moderately differentiated cervical SCC.MK and MD are correlated with SCC-Ag.
