硫酸镁联合西地那非治疗新生儿持续性肺动脉高压临床观察 被引量:11

Magnesium Sulfate Combined with Sildenafil in the Treatment of Neonatal Persistent Pulmonary Hypertension

摘要 目的:探讨硫酸镁联合西地那非治疗新生儿持续性肺动脉高压(PPHN)的疗效及其对患儿血氧饱和度和不良反应的影响。方法:选择我院新生儿重症监护室收治的新生儿持续性肺动脉高压患儿64例,随机分为观察组和对照组各32例。两组患儿均给予PPHN常规治疗,对照组采用西地那非治疗,观察组采用西地那非+硫酸镁治疗,根据患儿自身状况调整用药时间,连续治疗3~5 d,比较两组患儿的近期疗效及治疗前、治疗72 h后肺动脉压力(PAP)、氧动脉分压(PaO2)、二氧化碳分压(PaCO2)及血氧饱和度(SaO2),记录治疗期间药物不良反应发生情况。结果:治疗后观察组总有效率90.63%,高于对照组的78.13%(P<0.05);治疗后观察组PAP、PaO2、PaCO2、SaO2较对照组改善更明显(P<0.05);观察组不良反应发生率9.36%,低于对照组的18.76%(P>0.05)。结论:西地那非和西地那非联合硫酸镁均可治疗新生儿持续性肺动脉高压,其中联合用药可在不增加不良反应情况下可达到更好的治疗效果,且能明显提高患儿血氧饱和度。 Objective: To investigate the efficacy of magnesium sulfate combined with sildenafil in the treatment of neonatal persistent pulmonary hypertension( PPHN) and its effects on blood oxygen saturation and adverse drug reactions.Methods: Totally 64 neonates with PPHN admitted into neonatal intensive care unit( NICU) were extracted to be randomly divided into the observation group and the control group,with 32 cases in each group.Both groups received PPHN routine treatment,the control group was given sildenafil,and the observation group was treated with sildenafil combined with magnesium sulfate.The duration of medication was adjusted according to the children’s conditions,and continuous treatment was conducted for 3 to 5 d.The short-term efficacy,pulmonary artery pressure( PAP),oxygen arterial partial pressure( PaO2),carbon dioxide partial pressure( PaCO2) and blood oxygen saturation( SaO2) of two groups were compared before treatment and 72 h after treatment,and adverse drug reactions during treatment were recorded.Results:After treatment,the total effective rate of the observation group was 90.63%,higher than that of the control group( 78.13%,P<0.05).After treatment,the improvement of PAP,PaO2,PaCO2 and SaO2 in the observation group was more significant than that in the control group( P<0.05).The incidence of adverse drug reactions in the observation group was 9.36%,lower than that in the control group( P> 0.05).Conclusion: PPHN can be treated by sildenafil and sildenafil combined with magnesium sulfate.Combined therapy can achieve better treatment effects without increasing adverse drug reactions,and can significantly improve the blood oxygen saturation.
